Job summary

The Integrated Care Homes Service is looking for Band 6 Senior Staff Nurses to join the service. For registered nurses (two years post graduation along with community nursing experience) who are looking to further their career and develop new skills in leadership.

This vacancy is a 12-month fixed term/secondment position (until September 2025) working full time.

As a Registered Nurse, you will provide nursing interventions to a wide range of patients, by in reaching into Nottingham City Care Homes to deliver NHS services. The services overarching aim is to ensure that care home residents and staff have an improved experience through high quality care and access to familiar health professionals with the skills to meet their needs. This includes providing support to the staff within a care home setting in developing a plan of care for the patient.

The successful candidates will be required to have up to date clinical knowledge and skills and will be required to work autonomously, using initiative to deliver nursing care within a supportive team environment. They will be working with a diverse population providing support and nursing care to people who have complex health problems, with the aim to maintain and maximise their independence and prevent avoidable hospital admissions. You will be supported to learn and develop the skills required for this role.

The role of the Senior Staff Nurse is to provide nursing interventions to patients whose home is within a residential care home. This includes insulin administration, wound care, continence care, palliative support and EOL care.

As a SSN within this integrated service, you will be working closely with colleagues from dementia outreach and physical therapy facilitating your development of new skills and knowledge across all the elements of the service to enable a holistic approach to patient care, improving the patient outcomes.

The SSNs will work across Nottingham City supporting the 3 District Nurse Team Leaders with their caseload management, and staff supervisions. Whilst providing a high level of clinical support and guidance to junior staff and students

The service plays a key role in implementing the Enhanced Care in Care Homes Framework by providing support to care home staff, enabling them to deliver optimal care to their residents. This includes supporting staff within the care home setting to upskill and empower them to deliver optimal care to their residents. As well as developing communication pathways within care homes to enable proactive care and prevent avoidable hospital admissions.
