Job responsibilities

Job Purpose

To work as an autonomous nursing practitioner within a multi-agency setting in the provision of specialist advice.

To support the continuing care service head in the implementation, an assessment of the NHS funded nursing care and continuing healthcare policies following a multi-agency disciplinary approach within area of registration.

To provide support across Nottinghamshire and neighbouring ICB's and local authorities in the provision of health care assessment as required.

Act as an advocate for patient and carers in complex and vulnerable client group in partnership with local authorities and healthcare providers to ensure that client needs. Or met in the appropriate setting.

Clinical

Act as a role model to ensure high quality and consistency in policy application.

To work autonomously, as a health professional, in a local authority or health setting, to provide professionally accountable decisions and reports regarding clients eligibility for NHS continuing healthcare.

Responsible for carrying out health needs assessments and reviews for NHS Funded Nursing Care Contributions, including any formal reviews that may need to be referred to the Continuing Care Panel.

Coordination of Decision Support tool making sound and robust recommendations.

Analysing information from a range of assessments and information sources to ensure the health and well-being of the client is optimised.

Complete NHS Funded Nursing Care and NHS Continuing Care reviews in a timely manner.

Contribute to social work assessments to identify the nursing needs of clients with complex needs or those applying for long term care.

Continually review and improve systems which support the on-going monitoring, and review of client placements to ensure nursing needs are being met.

Provide information, training and advice, as necessary to lead clinicians, Local Authority Care Managers, Care Home Managers and other professional groups.

Work in partnership with social work colleagues and to advise on the most appropriate setting to address clients changing health and well-being needs.

Monitor the provision of evidence based/best practice.

Proactively raise issues and provide solutions in partnership with other colleagues in relation to change management, working across agencies to enhance care pathways and patient/carer experience.

Completion of Needs Portrayal documentation for retrospective reviews as required.

Presenting cases to the Independent Retrospective review panel as required.

Professional Development

To be responsible for the development of own knowledge and clinical practice and support ongoing professional development of team members.

Participate in the development of the teams clinical knowledge and specialist expertise.

Participate in the development of others, including students and carers.

To be responsible for the planning, implementation, evaluation, and development of student placements.

Participate in the supervision framework both as supervisor and supervisee.

Maintain a portfolio to demonstrate Continuing Professional Development.

Service Development

Contribute to the implementation of service and team developments.

Utilise enhanced skills and knowledge to promote continuous service improvements.

To be involved in the recruitment of new team members following robust selection procedures.

Support the Continuing Care Service Head in change management and continual improvement of the service delivery.

Contribute to the Clinical Governance agenda and apply risk management systems.

Participate in relevant research and audit activity.

Participate in peer review.

Management

Prioritisation of workload and delegation to team members to meet the dynamic needs of the service.

In the absence of the Continuing Care Service Head and Case manager be responsible for the operational management of the continuing health care assessment process, ensuring relevant commissioners are informed or the outcomes.

Team Working

Support and initiate effective team working both across and within Health and Social Care Teams.

Work closely with Continuing Care Service Head and colleagues to agree a consistent approach to implementation of the NHS Funded Nursing Care and Continuing Healthcare policies.

Support the motivation of team members and assist in the implementation of change management process.

Delegation

Responsible for the day-to-day delegation of activity to support nurses to ensure operational activity is maintained.

Patient and Public Involvement

Actively encourage patient and carer involvement in health needs assessments and at relevant multi-disciplinary meetings in order to complete the Decision Support Tool. To seek feedback about the service provided.

Help identify pathways of communication in the long term and continuing care process for patients and carers.

Record Keeping

Promote and maintain good practice in record keeping by self and the team, to ensure PCT and professional standards are met.

Complete electronic records of all Health Needs Assessments.

Keep a record of all activity, as agreed with the Continuing Care Service Head.

Produce professional reports for presentation at multi-disciplinary and inter agency meetings.

Participate in audit of assessment s and standard setting.

Ensure adherence to Caldicott guidelines and data protection act.

Health, Safety and Security

Promote and maintain best practice that ensures the health, safety and security of self, patients and colleagues in accordance with the policies, procedures and guidelines of Nottingham CityCare Partnership.

Practice in accordance with procedures and guidelines for the protection of vulnerable adults and child protection.

Ensure confidentiality is protected at all times.

INTERNAL

• Communicate complex, contentious information using the highest level of skill as it may occur in a hostile emotive atmosphere.

Maintain effective communication channels with patients, carers, colleagues, specialist workers and independent sector staff in a variety of settings across the Health and Social Care Community.

PHYSICAL EFFORT

All appointments will be subject to satisfactory medical clearance.

Required to attend monthly team meetings.

Ability to travel between locations within the Nottingham City and Nottinghamshire County boundaries.

Ability to work flexibly under pressure and subject to frequent interruptions.

Remain aware of client confidentiality.

WORKING CONDITIONS

Based at premises with the Continuing Care team.

Working across Nottinghamshire, in Nursing Homes and clients homes.

Within Nottingham City and Nottinghamshire County at an agreed base/location.

Essential

Experience

Essential

• Experience in a community role
• Significant nursing experience post registration in Learning disabilities, Mental Health or General Nursing

Desirable

• Experience in a supervisory nursing role working within a Learning Disability environment.
• Multi-agency working
• Demonstrates the ability to respond well to situations of conflict and maintain good working relationships
