Job summary
The Integrated Care Homes Service is looking for Registered Nurses with community nursing experience, or a student nurse who has completed their management placement in the community, to join our team. Various hours, fixed term and substantive contracts
As a Registered Nurse, you will provide nursing interventions to a wide range of patients, by in reaching into Care Homes to deliver NHS services. The services overarching aim is to ensure that care home residents and staff have an improved experience through high quality care and access to familiar health professionals with the skills to meet their needs. A single point of access enables the care home staff to refer patients directly to the service enabling the prioritisation and allocation of referrals to the most appropriate professional to meet their need
The successful candidate will require up to date clinical knowledge and skills and will be required to work autonomously, using initiative to deliver nursing care within a supportive team environment. They will be working with a diverse population providing support and nursing care to people who have complex health problems, with the aim to maintain and maximise their independence and prevent avoidable hospital admissions. You will be supported to develop the skills required for this role

Main duties of the job
The role of the Registered Nurse is to support the District Nurse in providing nursing interventions to patients whose home is within a residential care home. This includes insulin administration, wound care, continence care, palliative support and EOL care.
As a Registered Nurse within this integrated service facilitates the development of new skills and knowledge across all the elements of the service to enable a holistic approach to patient care, improving the patient outcomes.
