Job summary
An
exciting opportunity has arisen for an enthusiastic registered nurse to join
the Adult Bladder and Bowel Specialist Service. The team are part of Nottingham
CityCare Partnerships Adult Services, based at Sherwood Rise Health Centre to provide
specialist continence related care to housebound clients within Nottingham
City.
Incontinence is a pervasive
health issue and its impact on the quality of life for clients, their families
and carers are widely acknowledged. However, the fact that bladder and bowel
symptoms can be an indicator of more serious underlying health conditions, is
less recognised meaning every person who experiences bladder and bowel symptoms
should have their condition holistically assessed to understand the aetiology.
The successful
candidate will complete holistic continence assessments, translate outcomes
into individualised management and treatment plans, implement
care required and evaluate the effectiveness.
You must have a driving license
and access to your own transport.
The core hours of the service are 08.30 16.30hrs Monday to Friday.

Main duties of the job
Complete
holistic continence assessments and re-assessments, translate outcomes into
individualised management and treatment plans (including onward referral if necessary), implement
care required and evaluate the effectiveness for adult clients within a
community setting
To
provide continence advice to clients, carers and those who work with
clients with continence problems, through clinical practice, teaching,
research and development, as part of an integrated team,
Maintain contemporaneous records as an
autonomous practitioner.

CLINICAL
- To be professionally and legally responsible and accountable for all aspects of your work, including the management of clients in your care. This may include onward referrals depending on symptoms and clinical history,
- To ensure a high standard of clinical care for the clients under your continence management, and support staff to do likewise,
- To determine by assessment and liaison with other relevant agencies the advice, education and treatment requirements of each client and or carer,
- To undertake the comprehensive assessment of clients using investigative and analytical skills through standardised and non-standardised assessments,
- To formulate individualised management and treatment plans, using clinical reasoning and using a wide range of treatment skills and options to devise a programme of care and carry out treatment interventions,
- To accept clinical responsibility for a designated caseload of clients and to organise this effectively regarding clinical priorities and use of time,
- To provide advice, teaching and training to other members of the health care team regarding continence promotion and management,
- To have the ability to gain valid informed consent and to identify through assessment clients who lack capacity and work within a legal framework with clients who lack the capacity to consent to treatment,
- To work as a lone practitioner with supervision from band 6 and 7 continence advisor. The Team Leader will also provide clinical supervision.
- To provide spontaneous and planned advice, teaching and instruction to clients, carers and other professionals to promote understanding of continence treatment and management, and to ensure a consistent approach to client care.
MANAGERIAL AND LEADERSHIP
Effectively undertake line management responsibilities and provide support to Assistant Practitioners as appropriate.
Act as professional role model aligned with Trust values and behaviours and implementing a compassionate and coaching style of leadership.
EDUCATION AND DEVELOPMENT
To ensure knowledge, skills and competencies are kept up to date and complete all relevant competencies within recognised time frames as detailed in the competency guidelines.
To be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development in line with departmental policy. Also, to keep an active up to date CPD portfolio.
PROFESSIONAL
To participate in the development of good practice in continence care within Nottingham CityCare Partnership under the guidance of the Adult Bladder and Bowel Specialist Nurses and the Clinical Service Manager.
To support and encourage reflective practice in continence care, promoting a culture of continuous improvement.
- To demonstrate a commitment to seeking out and delivering evidence-based practice.
- To be responsible for own Continuing Professional Development under the supervision of the Adult Bladder and Bowel Specialist Nurses keeping abreast of new trends and developments, incorporating them into your work as necessary. Attend relevant conferences and internal/external training courses.
- To maintain a high level of nursing standards and work to the Nursing and Midwifery Code of Professional Conduct.
- To actively participate in the services training strategy, including in-service training, Trust and multi-agency training. To participate by attendance and presentation at staff meetings, tutorials and training sessions.
- To support the development, implementation and evaluation of established continence link nurse, worker schemes.
- To demonstrate a sound understanding of Clinical Governance and Risk Management and apply to the work situation.
- To assist with development and participation in audit projects, evidence-based practice projects and outcome measures, either individually or with other senior therapists. To make recommendations for change.
