Job summary

An exciting opportunity for an experienced Occupational Therapist to work within a specialist multi-disciplinary service has arisen with the CityCare Community Stroke team. The successful applicant would be responsible for the continuity and delivery of Occupational Therapy interventions for patients who are on the stroke pathway. You will be working with clients in their own homes, care homes and in our community discharge beds.

An understanding of stroke and /or neurological conditions, the physical, social and psychological impact of this long term condition on the patient and family is desirable.

This is an opportunity to join a supportive and innovative team and to be part of a wider network of Occupational Therapists within Nottingham CityCare Partnership

The successful candidate will join the other band 6 OTs in the team in providing:

Specialist Occupational Therapy Assessment and specialist interventions to support both patient, carers and wider team; promoting engagement with rehabilitation and self management.

Informal / formal supervision of other members of the service i.e Assistant Practitioners and RSWs

Teaching and ongoing assessment of OT competencies of support staff and students.

Main duties of the job

As an occupational Therapist in the community stroke service you will have opportunity to:

-Provide specialist assessment of patients functional abilities in their own home or other community setting, in order to establish treatment plans and person centred goal orientated plans, alongside an MDT including Physiotherapists, Specialist Nurses, Speech and Language therapists and support staff.

-Deliver specialist treatment plans, including provision of specialist equipment, neuro splinting, cognitive assessment and rehabilitation, upper limb rehabilitation, ADL related goals, and specialist postural management.

-Engage in development and training within the team, through supervision with Band 7 OT, in-service training and other CityCare wide training opportunities.

To be involved in service development and transformation of services to enhance service delivery for patients and their carers.

Clinical

3.To undertake the comprehensive assessment of patients, including those with a complex and/or chronic presentation using investigative and analytical skills through standardised and non standardised assessments. To formulate individualised management and treatment plans, using clinical reasoning and using a wide range of treatment skills and options to devise a specialised programme of care.

4.To set occupational therapy rehabilitation goals, carry out treatment interventions, occasionally as the only team member involved in the case.

5.To accept clinical responsibility for a designated caseload of patients and to organise this effectively with regard to clinical priorities and use of time.

6.To provide highly specialised advice, teaching and training to other members of the team regarding the occupational therapy management of patients around functional activities of daily living, equipment, specialist splinting, return to driving and cognitive issues.

7.To train, supervise and manage more junior staff, support staff and students. This will include the use of formal appraisal documentation and accreditation as a Clinical Educator. To supervise, educate and assess the performance of undergraduate Occupational therapy students as required to ensure the standard of practice and teaching meets the standards set by the degree level qualification

8.To have the ability to gain valid informed consent and to identify through assessment clients who lack capacity and work within a legal framework with patients who lack the capacity to consent to treatment.

9.To work as a lone practitioner with supervision from band 7 Occupational therapist.

10.To provide spontaneous and planned advice, teaching and instruction to patients, carers and other professionals to promote understanding of the aims of physiotherapy and to ensure a consistent approach to patient care.

1.To participate in the development of good practice for Occupational therapists within Nottingham CityCare Partnership under the guidance of the AHP Professional Lead.

2.To demonstrate a commitment to evidence-based practice.

3.To be responsible for own Continuing Professional Development under the supervision of the band 7 occupational therapist keeping abreast of new trends and developments, incorporating them into your work as necessary. 3.To liaise effectively with other professionals and agencies to enhance rehabilitation within the boundaries of Nottingham CityCare Partnership.

4.To manage the day-to-day delivery of the Occupational therapy service within own locality including prioritisation of support staff duties. Also organising own diary and time management. (Arranging assessments and appointments for own caseload)

5.To ensure the health & safety of staff and patients at all times in accordance with Health & Safety legislation and Trust policy and procedures. To adhere to all other Trust Policies and Procedures.

6.To attend team meetings and meetings with other agencies as required.
