Job summary

An exciting opportunity has arisen for a Support Worker position within our Older People's acute services to support our service users with all aspects of their care including activities and personal care.

Main duties of the job

This role focusses on the direct needs of service users by delivering individual treatment plans across boundaries of care.

To assist Care Co-ordinator or key worker and service user to assess, plan, implement and evaluate individual care plans and undertake other assessments as required.

  • Experience of working with people with Mental Health problems
  • An interest in and commitment to on-going personal/professional development

Knowledge

  • Basic IT knowledge

Desirable

  • A basic knowledge/understanding of the Mental Health Act and Mental Capacity Act
  • Use of SystmOne

Additional Criteria

  • Ability to respond to the individual rights/needs/opinions of service users
  • Ability to maintain a safe environment for service users/ carers/staff
  • Excellent written and verbal communication skills
  • Flexibility required to meet service needs
  • Team Worker
  • Positive attitude to change
