This is an exciting opportunity to work within a
friendly team to support the Neuropsychiatry services.

Main duties of the job
A Neuropsychiatry Support
Worker provides practical, emotional, and clinical support to individuals
living with neuropsychiatric conditions, helping them maintain independence,
wellbeing, and social inclusion. The role involves working directly with service
users in their homes, supporting families and carers, and collaborating with
professionals to ensure safe, coordinated care.
The postholder offers one to
one community based support, assists people to access health, social care, and
community services, and uses social prescribing approaches to promote healthy
lifestyles and reduce isolation. They contribute to clinical care by carrying
out cognitive assessments, physical observations, ECGs, and by using tools such
as the Older Peoples Thrive Menu to support personalised treatment planning.
A key part of the role is
liaising with families, carers, and multidisciplinary teams to ensure risks are
identified and managed appropriately.

- Social/Health care qualifications (including NVQ level 3 Care, Promoting independent Living) or equivalent
