Job summary

This is an exciting opportunity to join the Older People's Crisis and Home Treatment Team. They are a dynamic and busy team dedicated to providing high quality, person centred care in the either the community or inpatient settings.

Please note the expected start date for this position is not before 05/01/2026.

Main duties of the job

The Older Peoples Crisis and Home Treatment Team is part of the Acute Mental Health Service and ensures effective bed management, (including the gatekeeping) of all Older Peoples acute beds. The Post Holder will be responsible for the assessment, implementation and planning of care for the service users who are referred to the service.

Job description

Job responsibilities

The Post Holder will be expected to take responsibility for a number of tasks including:

1.Process referrals prioritise, risk management, risk assessment of clients who are in crisis and may be displaying high risk behaviours.

2.The Post Holder will be the first line contact for all mental health at all times to the general Trust and primary care via the Single Point of Access (SPA).

3.The Post Holder will be responsible for defining the level of acute mental health crisis and the use of resources. This includes the decision to admit a person to either the acute inpatient beds, or Sussex house crisis beds, processing of general practitioner referrals and urgent Acute Mental Health and Memory Service referrals.

4.The Post Holder will have to have an understanding of the relevant policies and their application to related face to face physical aggression and associated actions, eg COSHH, Infection Control, exposure to body fluids. Exposure to both verbal and physical aggression.

5.The Post Holder will be expected to professionally contribute and/or lead and manage serious untoward incidents, challenging behaviours, ensuring all parties are de briefed and supported.

6.The post holder will be expected to be involved in and implement new and revised policies within their own sphere of work.

7.Will offer clinical supervision to junior staff.

Essential

  • Knowledge of issues involved in the treatment of mental illness for older people and relapse prevention
  • Broad knowledge of risk assessments

Desirable

  • Knowledge of the principles of nurse prescribing and the use of service users group directives/protocols
  • Can demonstrate experience in audit and research

Qualifications

Essential

  • A current professional qualification (RMN, Social worker, Occupational Therapy)
  • Ability to demonstrate clinical leadership in a multi-disciplinary team setting
  • Evidence of post graduate training / studies

Desirable

  • Post qualification award in health/social care at degree or equivalent level
  • D32/33 or ENB998 (equivalent)
  • Clinical Supervision Training
  • Nurse prescribing
  • Degree level study
  • AMHPs trained/willingness to undertake training
