Specialist Young People's Mental Health Practitioner

Job summary

This is a unique opportunity to be part of the Mental Health Acceptance & Recovery Service (MARS) who work for 2 years with complex young people aged 16-25 years old who are vulnerable, i.e under the care system, looked after child, homeless, substance misuse, frequent attender to police, crisis etc.. The MARS team also transition young people at aged 17.5 years old from Young Minds Matters (YMM) to Adult Mental Health Services (AMHS), navigating pathways for on-going mental health support. We are very passionate about what we do and our approach is to work in a responsive, outreach, assertive style. Our ethos is to provide hope and work in an open dialogue approach working with families and young people, advocating on their behalf.

Main duties of the job

We are looking to recruit an enthusiastic, motivated, positive thinking, creative and proactive individual who has an inclusive outlook on life. You will be expected to work closely with YMM to ensure a smooth transition for complex cases. For this you will be expected to provide transition coordination as detailed in NICE guidance for transitions from YMM to AMHS. Clients may frequently self-harm or express distress in other ways which may challenge their loved ones, and put pressure on other services. As such you will need to remain calm and non-judgemental. You will need to engage clients who are at the beginning of their mental health journey and due to the stigma of having a mental health difficulty and by association being involved with mental health services may be difficult to engage. You will therefore need to have excellent interpersonal communication skills and a good working knowledge of caring for young people, building up trust.

Job responsibilities

Offer advice to a range of services within the wider community who may be a first place of contact for a young person with emerging mental health problems or their family e.g. schools, colleges, police, etc.

To promote the MARS ethos in all interactions with professionals, service users, their family and the wider community

You will need to be an excellent communicator with good time keeping and a positive mental attitude. You must have a passion for recovery, to help and support young people with complex needs. You will be exposed to challenging situations therefore it is important that you are able to self-reflect on your actions and attitudes and your own mental well-being and be able to manage your stress levels seeking, support supervision on a regular basis.

To work in a collaborative client-centred manner with both young people and their families/carers

To manage caseloads and administration systems

To assess risks and complete dices risk management plans assessments for young people, to collaborate with young people completing my care and support care plans, reviewing these using the dialogue and questionnaires to identify goals.

To work jointly and collaboratively with YMM, Psychology, Homelessness Team, We Are With You and other voluntary agencies to ensure the maximum service flexibility in meeting the young persons' needs to support all staff members within the team approach for difficult to engage and complex service users

To ensure maintenance of acceptable standards of practice and regularly evaluate these within supervision

To attend mandatory training courses and ensure compliance with professional competencies as required by Navigo and regulatory body.

To ensure that your work conforms to national and local policies and the relevant codes of professional conduct

To participate in clinical supervision as per Navigo policy

Maintain accurate and timely records on contacts with service users, their families and other agencies on the Electronic Patient Record (EPR).

Plan and organise own clinical diary.

Organise information gained from service users, notes and appropriate others to include in formulation of service user presenting problems

Understand and closely follow the MARS Operational Policy and other Navigo Policies.

Be critical in your approach to information to help develop an understanding of the whys in a situation to support a holistic and compassionate approach to service users.

Communicate complex clinical and non-clinical information in a non-judgemental and understandable way to a variety of stakeholders, service users and other agencies.

Ensure that confidentiality is maintained within the boundaries of relevant codes of professional conduct and legal responsibilities

Be approachable and a great active listener so you encourage engagement of young people with yourself and the service.

Be self-aware of own communication style and adjust as needed for individual service user needs.

Attend meetings such as ward reviews, child protection meetings, case conferences providing written reports as needed for your allocated service users, standing in for the team members as needed and for allocated lead roles.
