An exciting new opportunity
has arisen within The Community Rehabilitation Service.
The role of the Assistant Practitioner is to
work across the rehabilitation pathway (based primarily at Tees House),
supporting in the assessment of the health, social and wellbeing needs of the
individuals using the service as well as the delivery of appropriate
interventions accordingly. The successful candidate will be able to work both
autonomously as well as part of a multidisciplinary team (MDT), attending
reviews, MDT meetings and other meetings whether clinical or not.
The Assistant
Practitioner role will work under the supervision of the Clinical Lead and is
paramount in the safe, consistent and effective delivery of the service, also
providing supervision and mentorship to junior members of the team.
Strong
communication, organisational and clinical skills are essential, as well as a
solid value base and the ability to role model professional practice and
behaviour.
Main duties of the job
Provide
assessment based on rehabilitation need.
Plan and
ensure provision of appropriate treatment to promote physical wellbeing and support
mental health recovery.
Liaise with
colleagues, external organisations and stakeholders in order to co-ordinate appropriate
intervention, care and support.
Work within
scope of own training, experience, practice and applicable legal frameworks.
Demonstrate
and embed Navigo Code of Conduct through attitude, behaviour, leadership and
practice.
Take
responsibility for time management whilst balancing sometimes competing
priorities.
Lead shifts
and the team in the effective delivery of rehabilitation interventions.
Medications

Assess the
Health and Well-being needs of people within the rehabilitation service as
directed by the rehabilitation Care Coordinator
Assess,
implements and reviews activities and Care Programmes for individuals, planning
interventions including physical and personal care, day to day living
activities for example, budget control, shopping and medication, education,
training and employment, housing and accommodation issues
Following
completion of risk assessment and management training to complete comprehensive
assessment for risks associated with the individuals health and circumstances,
using recognised assessment tools including DICES (DICES risk management
training must be in date). To complete appropriate risk management plans to
minimise any risks to the service user and/or others and seek support if
required.
Undertake
daily review of Service Users health and social care needs as required against
the care plan, and take appropriate action to achieve the planned aims
To support
the documentation, monitoring and review of a range of specialist therapeutic
interventions for service users To deliver holistic evidence based care to
service users and their families using evidence based interventions and
assessment tools, seeking advice/assistance from senior colleagues as required.
To deliver
evidence based daily therapeutic groups and daily one to one sessions, ensuring
that these are planned collaboratively with the service users and the wider MDT
To monitor,
evaluate, and modify individual and/or group interventions in order to ensure
effectiveness and to improve service user therapeutic outcomes. This requires
observational skills, attention to detail and the ability to seek
support/advice from senior colleagues as required.
To liaise
with the multi-disciplinary team, care coordinators, general practitioners and
any other relevant parties involved, as per NICE guidelines and local policy.
Advise
service users, their family members and professional colleagues on appropriate
lifestyle changes to an individuals physical and social environments which
will promote enablement and independence.
To
participate in CPA reviews involving service users, their families and other
professionals and agencies.
To identify
when a carers assessment is required and to ensure referral on to appropriate
agencies and support networks (with support from senior colleagues).
To act as an
advocate for our service users and ensure that their voice is heard throughout
their care
To assist in
maintaining the safe administration and storage of medication in accordance
with Navigo policies and procedures.
You will need to demonstrate the proficiencies for medication
administration as set out in the NMC standards of proficiency and Navigo medicine management policy. Once signed off as competent by the Pharmacist and
having successfully completed your annual medicines administration competency
you will be expected to administer medicines in your working day
Prompt,
enable and encourage Service Users to be compliant with their medication regime
and support junior staff to do this under the supervision and direction of the
care coordinators. This may involve the use of concomitant aids, the collection
of medication from community or hospital pharmacies and the delivering of
medication to Service Users

organise workloads for staff on shift - to include allocation of daily jobs,
planning care and interventions such as group work and one to ones ensuring
that all are working to their care plans.
To review
and reflect on own clinical practice and performance through effective use of
professional and operational supervision and appraisal in line with local and
professional guidelines.
Demonstrate
compassionate and inclusive leadership to ensure that all staff feel valued and
effective and confident to speak out where needed.
Support
senior colleagues in the leadership of the service by speaking out and offering
your opinion regarding service improvement.
Support
development and maintenance of links with other agencies and external
colleagues.
