An exciting
new opportunity has arisen within The Community Rehabilitation Service .
The
Community Rehabilitation Service supports individuals in their recovery from
mental ill-health. The role of the Support Worker is to work across the
rehabilitation pathway (based primarily at Tees House), supporting in the
assessment of the health, social and wellbeing needs of the individuals using
the service as well as the delivery of appropriate interventions accordingly.
The successful candidate will be able to work both autonomously as well as part
of a multidisciplinary team (MDT), attending reviews, MDT meetings and other
meetings whether clinical or not.
Main duties of the job
This role is focussed around spending time
with our service users and as such it is a privileged role that gives you the
opportunity to make a real difference to peoples lives.
The
role will support the multi-disciplinary team to delivery individualised care
packages to a small group of service users with both mental health needs and/or
a learning difficulties, who are either stepping down from low secure and
locked rehabilitation placements or who are local service users requiring
assessment and treatment for their enduring mental ill health. The aim of the post is to support recovery,
independence, quality of life and well-being, through instilling hope and
optimism in every service user so that they can reach their full potential.

individuals with activities of daily living with the goal to enhance these
essential living skills.
To work without direct
supervision, but with someone available to refer to, as and when required,
either on or off site on a regular basis.
Help identify early signs of
relapse by monitoring the services user progress, level of functioning and
mental state and alert the appropriate staff involved in the persons care.
To support the service user
to integrate into the local community to enable them to feel a sense of purpose
and community.
To maintain informative
electronic records as required.
To undertake such other
duties as may be determined from time to time within the general scope of the
post.
Be able to work
flexibly within the different elements of the rehabilitation pathway when
