Job summary

A rare opportunity has arisen within the within Neurodiversity services. This post would be perfect for someone with an interest in supporting service users with neurodiverse conditions which are often found co-morbidly. You will work as an autonomous practitioner providing needs-led clinical care and required to use skills of person-centred collaborative working; assessment, and diagnosis; condition management and treatment.

The Adult Neurodiversity services working alongside CPG, are welcoming and friendly, share a common interest and passion for neurodiversity. The team have a wide range of skills and qualifications, based at Val Waterfront Centre.

Main duties of the job

You will draw on your experience and knowledge and administer needs-lead assessment, treatment and guidance, utilising evidence based assessment tools to support your clinical judgement in the assessment of adults with neurodiverse conditions. You will work as part of a team to identify what support/ needs are required for individuals referred into Adult Neurodiversity services, whether that be a diagnostic assessment, or signpost people who may benefit more from alternative/ appropriate services to meet their individual needs.

You may be required to build up a caseload of people, if this is identified (for example, ADHD). You may also support them with accessing groups/ support groups, or other NICE approved interventions offered.

You will be part of an MDT approach and support the triage and screening of referrals, discussions relating to diagnostic assessments and treatment/prescribing of medication.

Extensive experience in carrying out complex interventions, will demonstrate that these skills are maintained and shares those skills with other team members.

Complete developmental history and interpret with the aid of assessment tools for those referred for neurodiversity, following a needs-lead approach but still in line with the National Standards and NICE guidelines.

Work alongside the prescribing clinician and or Consultant Psychiatrist, to support safe and effective management with prescribing medication(s)

Participate in administration and interpretation of specific assessment tools with assessments and communicate the results to the Multidisciplinary Team.

Act as a specialist expert resource within the service in respect of this service user group working autonomously to support families and other health care professionals. To attend various multiagency meetings to advice, write specialist reports and plan care, acting as a patients advocate when appropriate.

Participate and potentially lead complex MDT Clinics to assess, diagnose, plan, implement and evaluate treatment/interventions and care for people with neurodiverse conditions.

Explores and analyses evidence, cases and situations in clinical practice, enabling a high level of judgment and decision making

Delivers advanced practice which is evidence based within service

Monitor physical health of service users on long term antipsychotic medications in line with NICE Guidelines and practice in line with the appropriate shared care pathways
