Job summary

An exciting opportunity has presented for a motivated and enthusiastic support worker to join the Carer Support Team within older peoples mental health service. The Carer support team are an integrated team that consists of Admiral Nurses who are supported by support workers, all of which work predominately work with carers who care for people with dementia as well as families that may be experiencing carer stress due to caring for a person with a mental health condition.

The team offers a person centred assessment of the carers needs and will provide information regarding the disease process and sign post them to other agencies and services that may be able to support them with their caring role.

Main duties of the job

Our support workers work very closely with our Admiral Nurse team who will guide and allocate the workload. This will consist of making regular visits with our service users as well as liaising with other teams and organisations to ensure that the best possible care can be provided for our Carers.

The support offered may be short or long term dependent on service user needs. They work with the carer towards identifying their needs and obtaining services to help them with their caring role. This may be pre diagnosis and / or may extend beyond a person with dementia passing away.

The post-holder will work with all members of staff within OPMHS and Navigo.

They will work in close liaison with health and social care professionals in statutory and non-statutory agencies to facilitate and deliver a range of services to ensure that continuing care needs of carers and/or service users are met.

Make contact with the carer and where possible check to identify whether the carer has input from the CarePlus Support Worker Service/ already has a Carers or Joint assessment/ intervention already in place.

Effectively communicate complex and/or sensitive information where there are barriers to understanding. This includes communication concerning adult protection, abuse, cognitive impairment and other risks surrounding a service users mental health. Barriers to understanding include working with service users who could be hostile or highly emotional, who have complex needs and may be vulnerable.

Review the carers needs on-going to ensure the support meets the carers needs and does not create the carer to become dependent on the Support Worker.

Maintain accurate clinical records according to professional standards.

To participate in Practice and Management supervision.

Promote and implement creative ways of working with carers and/or service users where necessary.

Instigate, develop and maintain a relationship with the carer and/or service user and/or carer to facilitate the accurate delivery of care.

Manage highly confidential information in respect of members of the public in accordance with local and national policy.

Experience

Essential

  • Work experience in Social/Health care (including housing support, welfare rights or equivalent)
  • Experience of working with older people with mental health conditions and/or people with dementia
  • Specific experience relating to carer support and/or assessment

Desirable

  • Experience in providing support to people in the community
  • Experience in working with people with Mental Health needs

Skills & Attributes

Essential

  • Able to organise own workload according to customers and service need
  • Able to work independently and use own initiative
  • Full driving licence and daily use of car

Desirable

  • Ability to negotiate work priorities
  • Skills in basic Information Technology
  • Ability to involve service users actively in planning and evaluating their care plan/package that promotes their independence

Qualifications

  • Social/Health Care qualifications (including NVQ level 3 Care, Promoting Independent Living) or equivalent or suitable dementia experience and willingness to work towards relevant qualification

Desirable
