Senior Rehabilitation Support Worker

Job summary

An exciting opportunity has presented for a motivated, enthusiastic and passionate individual to join our inpatient rehabilitation team.

Working with adults who have a diagnosis of a serious and enduring mental illness (SMI), this role is pivotal to supporting individuals to reach their full potential and optimum level of independence through the effective delivery of personalised and holistic assessment and treatment pathways.

Brocklesby Lodge is a mixed gender, 7 bed rehabilitation inpatient unit based at Harrison House, and is part of the wider rehabilitation pathway.

Main duties of the job

Developing therapeutic relationships, this role will support with the delivery of care plans for the individuals using the rehabilitation service. We use evidence-based, holistic biopsychosocial interventions, ensuring that the care we provide is person-centred.

This will include supporting with activities of daily living, developing skills and awareness, education & information, managing mental health and meaningful use of time, such as hobbies, interests, volunteering and employment.

The role will also provide support to junior colleagues, ensuring that the values and principles of rehabilitation are implemented in practice by everybody.

The post holder will benefit from support from senior colleagues, and ongoing learning and development is crucial to ensure the delivery of the highest standards of care.

Job responsibilities

From a position of understanding you will develop knowledge of the lived experience of people in your care so that you can provide the right support that will meet their own identified needs for recovery.

From a position of having a good understanding of mental illness advocate for your service users so that they are at the forefront of designing their own mental health care and support and so that their voice is heard at all levels

To develop supportive and caring relationships with individual service users and to provide daily support through encouragement, understanding and motivation.

To deliver prescribed 1:1 and group interventions with care and compassion, this may include but this list is not exhaustive:

I. Budgeting, benefits and welfare rights

II. Diet, exercise and well-being

III. Planning, shopping and cooking of meals

IV. Education, occupation, training and development

V. Healthy lifestyle, healthy choices

VI. Activities of daily living

VII. Rest, relaxation and vocation

VIII. Reporting on the effects of medication to senior staff

To provide daily updates to senior staff of individuals mood, behaviour, interaction, motivation and mental health so that plans of care can be reviewed accordingly.

Support junior staff to develop skills and experience of working within a mental health rehabilitation service.

Sensitively support individuals with activities of daily living with the goal to enhance these essential living skills

To work without direct supervision, but with someone available to refer to, as and when required, either on or off site on a regular basis.

Help identify early signs of relapse by monitoring the services user progress, level of functioning and mental state and alert the appropriate staff involved in the persons care.

To maintain informative electronic records as required

To undertake such other duties as may be determined from time to time within the general scope of the post.

Be able to work flexibly within the different elements of the rehabilitation pathway when required.
