Job summary

This is an exciting opportunity for an enthusiastic and motivated individual to join our Health and Care Partnership (HCP).A key priority for the North East Lincolnshire HCP and the Humber and North Yorkshire Integrated Care Board is developing Integrated Neighbourhood Teams and Proactive Frailty Care.

This role will be hosted in Navigo and work on behalf of the HCP

In this role you will operational lead the day to day activity management of these programmes working with stakeholders from across the community and HCP .

If you have programme management experience and a passion for improving services, and enjoy working collaboratively with people across different health and care settings then this could be the role for you.

Main duties of the job

A key national priority and for the North East Lincolnshire Health and Care Partnership and the Humber and North Yorkshire Integrated Care Board is developing Integrated Neighbourhood Teams and Proactive Frailty Care. These programmes of work will be a key focus for this role bringing the two programmes of work together and ensuring implementation across NEL.

The post holder will lead, plan, implement and report activities that drive this agenda forward.

The post holder will work closely with local communities and a range of key stakeholders across North East Lincolnshire.

Job responsibilities

1. Job Summary

Work alongside partners within the NEL Health and Care Partnership to lead specific service transformation programmes, with a focus on frailty and integrated neighbourhood teams

Work with colleagues to support the delivery of a range of other programmes and projects across the North East Lincolnshire Place, as required

Systematically engage with, and influence a range of providers and experts by experience to support the delivery of sustainable service change and integrated care, through effective stakeholder management

Work with providers and experts by experience to identify and minimise barriers to service development and/or gaps in service

Support the delivery of local health and care outcomes including positive service user experience and improved service quality; ensuring that these improvements reflect the local and national strategic aims

Be responsible for leading the delivery of specific service improvement areas/projects as defined. This will involve being the lead and subject matter expert for dedicated areas of work within agreed programmes

Provide a robust programme management approach to support the delivery of the key impact areas within the NEL Health and Care Partnership, working alongside the HCP programme team members

Working with complex provider arrangements, lead partners from across the HCP to develop the Integrated Neighbourhoods Team to deliver proactive frailty across NEL

To consider the impact of any change the projects will have on the system and support wide engagement across the system to secure full ownership and buy-in.

To ensure the flexibility of the projects, if required to meet conflicting/changing requirements

Day to day line management of project team members as required.

  • Extensive knowledge of specialist areas and knowledge relevant to the role acquired through masters level qualification or equivalent experience
  • Should have an appreciation of the relationship between different levels within the NHS and Care (e.g. DHSC, ICS, ICS, ICPs, NHSE)
  • Evidence of continuing professional development
  • Project Management qualification and/or training and/or experience

Desirable

  • Have an understanding of the background to and aims of current health and care policy in North East Lincolnshire
  • Member of relevant professional body
  • Training in tools and techniques designed to facilitate change
  • Post Graduate Management qualification

Experience

Essential

  • Experience of successfully leading change management initiatives and specific change projects
  • Experience of producing robust business plans
  • Experience of building and sustaining effective and productive working relationships with a wide range of people at all levels and across a wide variety of agencies and the public
  • Experience of working with independent contractors e.g. GP practices
  • Significant experience of public and patient involvement in service planning
