Job summary
Navigo and Panacea Primary Care Networks (PCN) are seeking an
experienced, highly motivated individual to join and support the practice
networks. This is an exciting
opportunity to join an emerging organisation and influence this new period of
development as a Primary Care Liaison Practitioner working as part of the
Primary Care Networks Multi-Disciplinary team across the member practices with
specific responsibilities to liaise with and support Primary Care colleagues to
ensure seamless care and assessment. The posts will be aligned to Panacea PCN
This role is to support assessment, intervention and signposting of the
community with Mental Health needs..
Main duties of the job
You will:
Provide
advanced triage process directly to GPs
Act as a liaison
point between primary and secondary care services
Work as an
active member of the PCN MDT undertaking supervisor role and perform
direct/indirect assessments.
Undertake
comprehensive assessments and deliver brief intervention care planning
Manage a
caseload of service users experiencing anxiety, depression or emotional
reactions that are not currently appropriate for management within secondary
care services
Support
those stepping down from secondary mental health services back to primary care
Promote and
foster a culture of active engagement and co-production across the PCN
community & system wide
Support the
Team Manager in the delivery of the integrated community mental health service,
ensuring it meets national guidance and local key performance indicators
Supervise
the Mental Health Navigator
Provide
in-direct supervision to the PCN aligned Mental Health Peer Support Worker
Actively
engage with the local VCSE providers as part of the Community Mental Health
Transformation programme, acting as the Community Mental Health representative
for the PCN

As part of
the Community Mental Health Framework and the integration into PCNs, this role
will pioneer the triage and assessment pathways for the PCN Team. You will directly liaise with GPs and act as
a single point of contact for the PCN where there is uncertainty about the
level of mental health need that requires further exploration. Following GP consultation, via direct booking
offering a triage appointment to determine priorities of need and risk and
coordinating onward referral to the most appropriate community service. You
will consistently monitor and track all referrals to reduce attrition, ensuring
clear feedback to the GP on progress for cases transferred into specialist MH
services.
As the
