This
is an exciting opportunity for an enthusiastic
and motivated individual to join our Health and Care Partnership (HCP) within
North East Lincolnshire to support the population health and Population Health
Management Programme.
North East Lincolnshire has a well established partnership
between our health and care providers; we strive to improve our joint working
so that local service users receive the best services possible.
In this role you will operational manage the day to day
activity of population health projects and lead our community centred approach
working collaboratively with local communities and key stakeholders from across
the community and HCP .

Main duties of the job
A
key priority for the North East Lincolnshire Health and Care Partnership and
the Humber and North Yorkshire Integrated Care Board is improving population
health, delivering population health management (PHM), reducing inequalities
and improving health and wellbeing of the local population.
North
East Lincolnshire has some neighbourhoods that are in the top 1% of the most
deprived in England and local communities that experience a number of
inequalities. We are developing our
approach to working collaboratively with communities to reduce inequalities and
improve outcomes, this role is instrumental in this programme of work.
The
post holder will work closely with local communities and key stakeholders
across North East Lincolnshire in the delivery of PHM Programme.
This role will be hosted in Navigo and work on behalf of the North East
Lincolnshire Health and Care Partnership.

Principal job duties / main tasks
The
post holder will be required to plan, support, facilitate and monitor progress
of designated workstreams, reporting progress and exceptions to the Programme
and other stakeholders as required.
The
post holder will be expected to lead engagement with a wide range of
stakeholders from communities.
The
post holder will be expected to engage relevant stakeholders across the Health
and Care Partnership in all aspects of the role.
To
be responsible for a high standard of work supporting the delivery of projects
on time, to quality standards and in a cost-effective manner.
Maintain
and update all project documentation with regular reporting to monitor progress
and resources to a range of forums and partners.
To
consider the impact of any change the projects will have on the system and
support wide engagement across the system to secure full ownership and buy-in.
To
ensure the flexibility of the project, if required to meet conflicting/changing
requirements
Day
to day line management of project team members as required.
The
role supports the Health Care Partnership and Humber and North Yorkshire
