The
Community Rehabilitation Team promotes person-centred care with the objective
of supporting individuals to reach their full potential and to achieve the
greatest level of independence relative to their needs and circumstances. The Senior Rehabilitation Practitioner requires a
candidate who can demonstrate compassion, kindness and resilience, as well as applying
their previous experience of effectively working with individuals with a
diagnosis of an SMI, they will be responsible for the care co-ordination and
risk management of individuals throughout the rehabilitation pathway as well as
providing 1:1 and group interventions to the rehab pathway. The successful
candidate will role-model not only to the individuals using the service, but to
junior staff members in order to progress with their development and improve

Main duties of the job
To
supervise and coordinate the clinical care of a defined caseload.
Assess,
plan, implement and evaluate care to provide appropriate interventions to meeting
individual need.
To
assist in providing professional leadership, guidance and advice to junior team
members and other members of primary and specialist services.
To
maintain and enhance close professional working relationships with referring
agencies and colleagues from other disciplines and the voluntary sector,
offering professional guidance and advice in relation to Mental Health issues.
To
deputise for the Team Manager, Clinical Lead, in their absence and represent
the team / service as required.
To
provide specific interventions to the rehabilitation service individuals via
group and 1:1 work.
To
commission services under the Care Act 2014 when and individual is assessed as
requiring these

Job responsibilities
Manage
a caseload utilising patient centred assessment, planning and implementation of
care plans.
Evaluate
care plans and outcomes including active participation in regular clinical
audit.
Complete
Risk management plans and assessment as required
Liaise
with other members of the multi-disciplinary team thereby maintaining and
enhancing close professional working relationships.
Participate
in regular clinical supervision in accordance with the Navigo Policy for both
clinical and managerial aspects of their work.
To
record and maintain all professional, clinical information of patient care
needs in a confidential manner.
To
work within the guidelines for Professional Practice / Navigo Policy.
Carry
out the requirements of the Mental Health Act 1983, Care Act 2014 and the
Mental Capacity Act 2005.
Adhere
to local policy regarding storage, carriage and administration (nurses only) of
prescribed medication.
Keep
abreast of professional development and updating requirements.
To
actively link in with the Primary Healthcare Team, the Multi-Disciplinary Team
and Care Programme Approach reviews.
To
work with individuals and families at a variety of venues including clients
houses, hospital bases, GP surgeries etc.
Participate
in the service duty system as required.
To act
as Care Coordinator under the Care Programme Approach
Acts
as an Approved Social Worker/Approved Mental Health Practitioner if qualified
to do so.
Undertake
specific related tasks commensurate with professional background
Act as
a Best Interest Assessor if qualified to do so.

guidance/codes of practice.
