Whether you have experience working within Crisis/Liaison/SPA services, or have experience in another mental health setting and are looking for your next challenge - this role plays a pivotal part in improving the lives of the people using our services, supporting them to access the most appropriate service(s) for their needs. Main duties of the job
The Access Team Practitioner will work within the integrated Access Team, covering Crisis, Hospital Liaison and SPA. You will be responsible for initial assessment, implementation, planning, and evaluation of care. Assessment of risk along with subsequent management and safety planning is a significant part of this role, including face-to-face and over the telephone. Covering a 24/7 rota, the post-holder will provide essential mental health assessment and support, whilst sometimes overseeing and supervising other areas of acute services and acting as site lead. The role will entail both team working and autonomous working practices, together with personal responsibility for caseload management from first contact to discharge. You will be an inspiring supervisor of junior colleagues and students, also delivering training/information to the team and wider services. The post-holder will work closely within the multi-disciplinary and inpatient team along with carers/family members.
