Will the Navigo Access Team be the next step in your career?This is an exciting opportunity for an experienced, qualified staff member to join a multidisciplinary and dynamic team of individuals within The Access Team. Within the Access Team is the crisis team, Hospital Liaison Team and Single Point of Access. This role will require you to work across all three of the areas. You will also be joining the team at an exciting time in Navigo Acute services as things are changing and developing to ensure we continue to deliver outstanding services to the population of North East Lincs. There will be numerous opportunities to develop your skills, undertake training in psychological interventions including the innovative CAMS program (Collaborative Assessment and Management of Suicidality). Main duties of the job
The Access Team Practitioner will work across the Crisis Team, Hospital Liaison Team and Single Point of Access. You will have responsibilities including initial assessment, implementation, planning, and evaluation of care. Assessment of risk along with subsequent management and safety planning is a significant part of this role- face to face and over the telephone. You will work on a 24 hour rota which includes weekends and nights. You will act as site lead out of hours which includes overseeing and supervising other areas of acute services when required. The role will entail both team working and autonomous working practices, together with personal responsibility for caseload management from first contact to discharge. You will be required to manage and supervise junior staff and students, also deliver training/information to the team and wider services when required. The Post Holder will work closely within the multi-disciplinary and inpatient team along with carer/family.
