Job summary

This is an exciting opportunity for you to join a multidisciplnary and dynamic team of individuals within the Crisis Home Treatment Team. You will be joining the team at an exciting time in Navigo Acute services as things are changing and developing to ensure we continue to deliver outstanding services to the population of North East Lincs. There will be numerous opportunities for you to develop your skills, undertake training and be part of a team who are about to commence formal accreditation with the Royal College of Psychiatry. Main duties of the job

The role will be to work as part of the Crisis Home Treatment Team (CHTT) focussing on the direct needs of service users. Home treatment duties will include developing and delivering individual treatment plans across boundaries of care, working closely with qualified members of the team within CHTT and across the acute service. There will be opporitunities for training and development as we build our home treatment service in order for us to deliever a true alternative to inpatient admission. On occaison you may be required to perform some of the frontline duties required within the CHTT and these include acting as an initial point of contact when and individual attends Harrison House requesting to see the crisis team, ascertaining need and level of support required, documenting and ensuring this information is relayed to qualified staff.

Job responsibilities

The main duties and responsibilities of the Home Treatment Support Worker include:

To work without direct supervision, but with someone available to refer to, as and when required, either on or off site on a regular basis, and to understand and work with a level of autonomy, making judgements around patients conditions, at that time, and their potential requirements..

Ensure that service users engage effectively with the agreed care plan and across appropriate services provided on a regular and consistent basis.

Provide regular and practical support to service users and their carers/significant other in developing and managing dignity and independence.

Provide support and assistance, as necessary, with daily living activities.

Positively promote independent living of service users within the community.

Work flexibly with Key Worker, Care Co-ordinator, other relevant community support, service user and significant other to implement plan of care.

Help service users gain access to resources, including benefits and welfare rights and other potentially useful outside agencies.

Have a sound knowledge of risk management and potential escalation procedures.

Help identify early signs of relapse by monitoring the services user progress, level of functioning and mental state and alert the appropriate staff involved in the persons care.

Assist the Nursing Team , Care Co-ordinator/Key Worker and service user in developing crisis/contingency plans and advanced statements, as per local policy guidelines.

To maintain adequate records, as required, by existing procedures, entering appropriate details on the service users case notes, as necessary on silverlink

To be able to communicate routine information in an arena where there will be barriers to understanding, and to be able to use tact and persuasion in a non-aggressive manner.

Provide information on health promotion.

To ensure the service user understands and has a clear pathway of care across sector/agency boundaries with key contact points/named individual.

Attend and participate in agreed training, development and supervision sessions as an individual and as part of the team.

To undertake such other duties as may be determined from time to time within the general scope of the post.
