Job summary
The Urgent Care Team is a 24/7 service; providing a first
point of contact and triage service, including telephone and face to face
clinical assessment of patients and/or their carers/relatives, resulting in
appropriate decision making and plan of care. The Team work closely with other
disciplines including GPs, District Nurses, Mental Health Services, Adult
Social Care, End of Life Care Services, Paramedics and other external
providers.
The Triage Clinicians, as part of the Urgent Care Team, are
a dynamic group of Nurses and Paramedics with a diverse range of experience and
backgrounds, which responds to the unplanned needs of the patients in North
East Lincolnshire.

You will be required to work a variety of
shifts including evenings, weekends and Bank Holidays
Salary Range 6 £35,527 - £42,663 pro rata per annum
We are a friendly, supportive team of clinicians providing a
Clinical Assessment Service to the residents and visitors to North East
Lincolnshire. We play a
pivotal role in the support of community healthcare and hospital avoidance. We
are a unique service being able to provide a direct access point for both
professionals and members of the public with all clinicians having an understanding
of the area/services available and social/economic considerations ensuring a
holistic approach is maintained. The service is proactive and works closely
with other organisations to pilot new, innovative strategies.
By utilising agreed protocols and policies in conjunction
with listening, questioning and probing skills you will support in planning
individual patient care. You will complete clinical assessment of minor
ailments through to life threatening emergencies and arrange appropriate and
safe Ambulance and GP attendance, if required.
