Job summary

Palliative and End of Life Care Services provide various of levels of support to people with limited conditions and those who are in the final phase of life. The service provides, one to one support as well as advisor capacity to other professionals. The service will work through a central point of referral, working in conjunction with the Community Nursing Teams.

The Palliative Care Team is a health and social care team that provides an expert clinical and social care service for patients with challenging and complex life limiting diagnoses and their families. All senior team members act as clinical or social care experts, leaders and educators for their service areas. The team provides a service in all care settings across the North East Lincolnshire locality.

Main duties of the job

Job title: Clinical Nurse Specialist in Palliative Care

Department: Palliative and End of Life Services

Base: St Andrews Hospice

Hours: 30 hrs a week

Salary Range 7 £41,617£47,499 pro rata per annum

The successful applicant will work autonomously in undertaking specialist palliative care assessments and provide specialist clinical advice, intervention and support to people with complex palliative and end of life care problems. Delivering the service across all care settings which will include community, hospital and care homes as appropriate and work in collaboration with cancer and palliative care teams in North East Lincolnshire.

You will be responsible for the prescribing of appropriate palliative care drugs to patient need and act as an expert educator as part of the Palliative and end of life care education programme, to staff from all care settings in NE Lincolnshire.This also involves leading and continually supporting the service development in areas of designated responsibility and providing leadership, innovation and excellence in all aspects of the role.

You will be a First Level Registered Nurse, a relevant degree, a formal teaching qualification, ENB 285/237 or equivalent, Advanced communication skills training (or a willingness to undertake), Non-medical prescriber or willingness to undertake and Psychological training level 2.
