Job summary
The
Macmillan Specialist Palliative Care Team is a health and social care team that
provides an expert clinical and social care service for patients with
challenging and complex life limiting diagnoses and their families. All senior
team members act as clinical or social care experts, leaders, and educators for
their service areas. The team is also part of the integrated support, welfare
and bereavement service across the end-of-life care partnership providing a
service in all care settings across the North East Lincolnshire Locality.

We are looking for an experienced, qualified, proactive and forward-thinking social worker to join our dedicated multi-disciplinary
palliative and end of life care team. Working alongside the Specialist Palliative Care Social Workers, you will assist patients and their family/carers
with various aspects of care before and after death. You will participate in MDT meetings, case reviews, complete mental capacity
assessments and chair best interest meetings and make referrals for adult or
childrens safeguarding as required.
You need to demonstrate use of initiative, clear and reasoned
decision making, advanced communication skills and the ability to
work independently and as part of a wider team, developing and maintaining
external agency partnerships to improve delivery of patient care. You will also need the ability to
visit individuals across all settings, including their own homes.

multi-disciplinary team committed to delivering high quality care for
individuals and their family pre and post bereavement. You will receive
