Main duties of the job

Job Title: PCN Care Co-ordinator

Hours: 29

Monday 8.00am - 6.00pm, Wednesday 8.00am - 6.30pm, Thursday 8.00am - 6.00pm

Fixed Term to cover maternity leave for 9 months

£23,603 fixed salary point Pro Rata Per Annum

You will work closely and in partnership with the Primary Care Network clinical team, coordinating resources and information to support choice, access and delivery of personalised care.

Care Coordinators will be responsible for consulting with patients and determining their needs, developing care plans, coordinating patient care services, education regarding their conditions and working with the medical team to evaluate interventions, holding group activities.

Consult with patients and family members to discuss their health and social problems including providing support, I.E housing benefits. Support patients in their understanding and management of their condition and medications, within the remit of the role. Develop care plans to address personal health care needs.
