Job summary

Care Plus Group Primary Care practices provides the first point of contact for registered patients accessing the health care service. The service will work collaboratively with patients to assist them in managing and preventing illnesses and coordinating access to specialised care.

The Care Plus Group Primary Care practices, provide services for a client group with a range of diverse and sometimes complex needs including substance misuse, homelessness, mental health problems and self-neglect. The Care Plus Group Primary Care practices are part of the Meridian Primary Care Network (PCN).

Main duties of the job

Social Advice Officer

CPG Primary Care

Based at: Open Door and Quayside Medical Centre

30 hours - fixed term contract for 18 months

£20,375 pro rata per annum

You will supervise the Drop-In Advice and Support service and ensure compliance with all policies and procedures and maintain excellent customer service.

You will co-ordinate student placements (social care students and student counsellors); and ensure that they receive induction training and comply with Open Door protocols and processes.

You will work within a team to provide support, advice and guidance to service users/clients, who attend Open Doors Drop-In Service.

You will respond to emergency/crisis support needs and on-going needs of clients requesting help via Drop-In Service, telephone, referral from other agencies or requests from Open Door staff (Social support or health/medical staff) for assistance in helping their clients through crisis situations.
