Job summary
Intermediate Care @ Home delivers a service of high quality and excellence over 7 days a week; 365 days a year providing support for people in their own homes within the boundary of North East Lincolnshire.
The success of the service reflects how we work within a robust partnership inclusive of integrated health care professionals. Intermediate Care @ Home provides intermediate health and social care support to service users within an enablement ethos. Staff will use appropriate knowledge and skills to report changes in condition to the appropriate professional.
Following appropriate training, duties will include personal support, clinical duties, therapy programmes; social care support and some practical duties to enable and improve daily living skills.
The Apprentice Integrated Support Worker will work in partnership with nurses, physiotherapists, occupational therapists or care managers across the intermediate tier service and undertake delegated duties after appropriate instruction in conjunction with a detailed support/outcome plan. All staff will be subject to the enhanced Disclosure and Barring Service.

Based at The Val Waterhouse Centre, Grimsby but involves community working within North East Lincolnshire
Various hours available (between the hours of 7am and 11 p.m. on a flexible rota including week-ends and Bank Holidays)
Fixed Term for 18 months
£10.42 per hour (age 23 and over)/£10.18 per hour (age 21-22)/£7.49 per hour (age 18-20)
We are seeking to recruit a committed member of staff to work within an established and integrated enablement service with an excellent reputation.
Providing short-term high quality services to people within their own homes across North East Lincolnshire, the service is dependent on having a robust multi-skilled team responsible for ensuring we get the best possible outcomes by working in partnership with service users; carers; families and our health colleagues.
The role also includes working with therapy programmes and always ensuring that we promote an individuals independence at all times.
