Job summary

If so, we have a full-time vacancy for you within our Hospital Palliative Care Team. We are looking for a Band 8a ACP who is resilient, compassionate, resourceful and a good team player.
Main duties of the job
The post holder will work as an autonomous practitioner within the specialist palliative care service, based predominantly within the Hospital Palliative Care Team, at Medway Maritime Hospital Foundation Trust. The role involvesa high degree of autonomy and complex decision making. This is underpinned by a master's level award or equivalent that encompasses the four pillars of advance practice:
Clinical practice
Leadership and management
Education
Research,
The post holder will have the ability to manage the patient's complete clinical care in partnership with individuals, families and carers. The post holder is expected to be able to complete a complete medical and holistic assessment of the patient. Subsequently they will be expected to make decisions regarding diagnosis, interventions and prognosis in a context of complexity, uncertainty and varying levels of risk, being accountability for their decision making thus managing the complete care of the patient.
The ability to manage the patient's complete clinical care includes assessment, diagnosis and management of reversible conditions, management of pre-existing long-term conditions as well as the assessment, diagnosis and management of the presentation directly linked to the end of life. This is supported by a wide scope of practice with regard to prescribing.
