Main duties of the job

We are looking for clinical skills facilitators to work with and support the practice educator in these exciting new roles, to develop, maintain and support learning, development and growth within our neighbourhood nursing teams using evidence-based practice.

You will work in close collaboration with all members of the neighbourhood nursing teams, to provide comprehensive, evidence-based practice, assessing the quality of a range of practice learning environments, determine interventions to sustain and enhance practice education; ensuring all localities have an up to action plan in place. You will support and facilitate the development of clinical supervisors, assessors and preceptors, providing supervision and support structures in order to build education in practice capability, consistency and efficiency.

Job purpose

To work and support collaboratively with the Practice Educator to develop, maintain and support learning, development and growth within the Neighbourhood Nursing Teams using evidence-based practice.

To work in close collaboration with all members of the Neighbourhood Nursing Teams, to provide comprehensive, evidence-based practice.

To assist Neighbourhood nursing Teams in working across all localities

Assess the quality of a range of practice learning environments, determine interventions to sustain and enhance practice education; ensuring all localities have an up to action plan in place.

Support and facilitate the development of clinical supervisors, assessors and preceptors, providing supervision and support structures in order to build education in practice capability, consistency and efficiency.

To support the Practice Educator to develop, co-coordinate, evaluate and implement education programmes, contribute to development, validate and review through an annual quality review.

Key responsibilities

To deputise when required in the Practice Educators absence and delegate work appropriately and attend meetings as assigned.

Identify opportunities and work collaboratively with Practice Educator and Locality Leads on the planning, structuring, design, delivery and review of inter-professional learning opportunities and education in practice for health care learners, supervisors, assessors and preceptors.

Collaboratively identify the learning learns needs of different professional groups within Neighbourhood Nursing Teams, ensuring that the facilitation of inter professional learning outcomes are met as per educational policies, Standard Operating Procedures and Clinical Competencies.

Provide best practice advice in the clinical setting for practice assessors, mentors, preceptors and staff in relation to education in practice.

Develop and maintain up to date, accurate records and live register of practice assessors and preceptors in the practice setting.

Monitor staff performance inline with MCH policies, SOPs and clinical competency and work collaboratively with the Education Team, Locality Leads and staff member to produce action plans, if necessary, to support development.

To be responsible for organising and prioritising own workload in the day.

To actively promote self-management which enables the patient to proactively manage their long-term condition and know how to confidently manage or respond to any expected exacerbation or deterioration in their condition.

To work closely with other members of the team, provider services, health and social care partners and GPs to identify high risk patients providing a holistic assessment and assurance that they are able to access the relevant services.

To support the staff member to complete holistic assessments of patients referred to Neighbourhood Nursing and liaise with the Neighbourhood Nursing Teams on patients treatment needs.

To assist in monitoring the quality of nursing services offered and contribute to the development of quality measures.

Identify the need for and participate in clinical audit, implement change in clinical practice and participate in monitoring and evaluation processes.

To undertake the teams appraisal and performance development planning process, identifying training needs of team members, ensuring appropriate training is provided and that training is utilised in practice.
