We are looking for a care manager assistant to work in the urgent response care management team. The successful candidate will be working towards preventing hospital admission, promoting independence and facilitating early discharge. The successful candidate will need to have excellent communication skills, knowledge of the health and social care environment, and outstanding organisational and IT skills. The role will involve visiting service users in their own homes and completing regular reviews of care.

Main duties of the job
The post holder will work alongside the other care manager assistants to provide an effective duty, and triage system for new referrals coming into the UR CM Team.
Good keyboard skills required for information recording, report writing and e-mails.
The post holder will manage a caseload of service users , to monitor and review care services in the community. Ensuring that the cost of the care is appropriate to the service users needs.
The post holder will be responsible for completing reviews at the end of the enablement period, to ensure that service users with long-term needs have ongoing support in accordance with The Care Act 2014.
The post holder must be able to easily access different properties and environments including house boats, rural setting and cluttered environments.
The post holder will need to be able to appropriately and professionally engage and communicate with a wide range of service users from different health, social and ethnic and cultural backgrounds
