Job responsibilities
Provide expert clinical intervention by promoting timely and appropriate decision making and assessments for patients
Participate in the writing of reports in an accurate thorough way using IT
The post will involve liaison with other intermediate care services within the Medway area, primarily the community rehab teams but also our Council colleagues
Work with multi-disciplinary team to facilitate timely and appropriate clinical management of patients
Develop and maintain good working relationships with inpatient and community services within the organisation and with external bodies
To work as a key member of the transfer/discharge process within Medway, ensuring effective communication and developing sound relationships with colleagues, health and social professionals from within and external to Medway Community Healthcare, as well as colleagues in the voluntary and private sector
Provide clinical leadership and act as role model for all staff
Registered nurse or Therapist with evidenced relevant experience such as previous A&E, community or Nurse in charge experience
Extended nursing skills to include:-
Registered nurse 1st level/Therapist or educated to degree level
Evidence of continuous development
Obtain and interpret patients observations
Know principles of infection control
Sound clinical knowledge and skills in the support of immediate management of patients requiring clinical intervention
Evidence experience in identifying and managing unwell patients
Experience of multi-disciplinary working
DOLS
Able to work flexibly and cooperatively as well as autonomously
Confident with decision making
Confident in the MCA 2005
Possess a working knowledge of IT software programmes and how they function, with a good standard of inputting skills
Evidence of an awareness and understanding of national targets both in health and social care
A willingness to further develop skills including clinical and leadership possibly through degree modules.
Sound knowledge of MCH services available in the community.

Ad-hoc management of in-patient areas and Medway Hospital site to ensure patient and staff safety
To assess practice and standards on the wards and in the community of Infection Control Precautions and to ensure the organisations policy is applied at all times

Using the Hospital Bed Management system, social services and TOCH database.

Co-ordinate the transfer of patients, including those with complex health and social care needs, between the Medway foundation trust and Medway Community Healthcare.
To assist the Clinical Lead and Discharge Coordinator to manage staffing issues across the team providing senior nurse support and guidance. Deputise for the band 7s.
Provide professional leadership developing teaching and supervising staff, patients and their families to raise understanding, standards of professionalism and contribute to the development of nursing.
To carry out staff PDRs, 1-1s, informal sickness reviews up to and including stage 1 and manage a small team of band 4 and 5 staff.
Good computer skills in Word, Excel, Outlook and other software packages.
Patient moving and handling, including transfers patient from bed to chair and similar in order to support ward staff.
Be able to travel to dispersed sites in a timely manner if required.

Actively promote an effective learning environment and participate in the education and professional development for nurses across the hospital during; out of hours.
In conjunction with the Discharge co-ordinator and manager on call, use and deploy staff resources to achieve optimum value for money without detriment to patient care.
To be signatory and sign off travel/payments for agency/bank time sheets and other ad hoc expenses with the authority of the ward co-ordinator
Understanding of resource/budget management
To effectively manage the area in accordance with available financial resources
Handles patient valuables and property in accordance with hospital and MCH policy
Act as a mentor and facilitator for staff in the team
Use the appraisal system to identify training and development needs of all staff
Facilitate and participate in training and education programmes, promote an effective and efficient discharge process

Organise and deploy staff according to competencies and workload
Assist the Discharge co-ordinator and on call manager in liaising with outside agencies to request appropriately trained staff when shortfalls arise
Provide psychological support for team members, patients and their families
The post holder is required to maintain confidentiality of information regarding patients, staff and other health service business in accordance with the Caldecott guidelines, Data Protection act and Childrens Act and other relevant legislation as appropriate
Information Governance policies
To maintain accurate patient and staff records and ensure systems, both electronic and paper based are adhered to within organisational policy
Ensure appropriate data and information is collected, collated and communicated to appropriate personnel in Medway Community Healthcare and the Strategic Health Authority. This includes current bed state situation, weekly delayed discharges and maintenance of the rehab list for community hospital beds
Assists with and facilitate data collection programmes such as clinical audit by the use of agreed tools
Participate and implement organisation / hospital / ward audits and surveys
Ability to implement new projects
Prioritising workload, assessing ill patients and making effective clinical decisions and referrals to appropriate teams
To work unsupervised and independently managing own workload
To comply with own professional registration guidelines and codes of conduct
To be the co-ordinator dealing with the transfer and discharge of patients within Medway Community Services
Using the Organisational policy, will act independently in advising on clinical practice to organisation staff
Work in accordance with hospital and departmental policies, procedures and guidelines, and has significant discretion to work within a set of defined parameters
Is competent with current emergency resuscitation techniques, fire procedures and emergency plan both in theory and practice and is expected to take a key role in these emergencies.
