Job summary

  • Are you an experienced registered nurse with drive and enthusiasm?
  • To work as a key member of the transfer/discharge process within an acute Hospital, ensuring effective communication and developing sound relationships with colleagues, health and social professionals from within and external to Medway Community Healthcare, as well as colleagues in the voluntary and private sector.
  • Are you able to act as a resource to facilitate a whole system approach to the transfer/discharge process and promote a positive experience for the patient.

Main duties of the job

We are looking for an enthusiastic, motivated nurse who demonstrates good interpersonal skills and is willing to work flexibly within the Integrated discharge clinical team. Our aim is to support with the holistic assessment of patients in a hospital environment and create a personalised plan of care for all patients once assessed and to provide expert clinical intervention by promoting timely and appropriate decision making and assessments for patients.
