We are looking for a registered professional who is passionate about dementia care and would like to make a difference.
The dementia crisis team are a home treatment service for dementia patients in crisis in Medway, the service is intended to enable people to remain in their own homes and to reduce unnecessary admission into Medway Maritime Hospital and we support discharges from hospital back to the community and to care homes.
The team work with patients with physical health problems or changes in behaviour that surface as a result of other health problems or as advanced stages of their dementia.
Main duties of the job
- To work as a member of the Dementia Crisis Support Service, supported by senior staff to provide specialist primary healthcare and mental healthcare nursing assessments, programmes of care and skilled dementia care and advice to patients, carers and staff
- To carry out nursing procedures as deemed appropriate by senior, experienced members of the multidisciplinary team
- To maintain timely and accurate clinical records
- To provide clinical supervision and support to other staff members and students as deemed appropriate
- Assist in the ongoing development and evaluation of the service

- Ability to effectively communicate with staff, patients and their families, carers, managers and staff from other services and agencies, members of the public, private and voluntarysectors

2. Knowledge, training and experience
- Registered Mental Health nurse or Registered General nurse
- Community nursing experience
- Ability to make decisions and to accept personal accountability
- Able to work as part of a multi-professional team with knowledge and understanding of the roles of other members
- Understanding of recent issues and developments which may have an impact on the delivery of health services for people who have dementi
3. Analytical and judgement skills
- Able to analyse facts and options to enable professional judgements/assessments to be made. This may include decisions relating to ensure effective and person centred care
- Proactive management of risks
4. Planning and organisational skills
- Able to assess, plan, implement and review patient care, ensuring an individual and holistic approach is achieved and that agreed standards are maintained at all times
- Able to prioritise and respond appropriately to patient referrals
- Able to effectively manage own caseload and take responsibility for own day to day workload planning, including allocation and delegation of work to other team members
- Support the senior clinicians in the management of the caseload and smooth running of the team

- Ability to drive or be independently mobile due to the need to travel between varied locations during the working day.
6. Responsibility for patient / client care
- Undertake the nursing care of patients as appropriate ensuring individual nursing needs are met through the use of evidence based practice
- Assess, plan, implement and evaluate programmes of care in consultation with senior clinicians
- Continually reassess patient progress, adapting plans of care accordingly, and develop discharge plans
- Play an active role in weekly MDT meeting
