An exciting
opportunity has arisen to recruit to the post of band 5 Community Learning Disability
Nurse to join Your Healthcares Neurodevelopmental Service. We would welcome
applications from registered nurses who have a special interest in working in learning
disability services or those approaching the completion of their Nurse training.
This team is ideally equipped to support you develop your skills and knowledge
in this field.
The Neurodevelopmental Service provides a comprehensive
range of specialist community services for adults with learning disabilities
within a multidisciplinary healthcare team. We also provide assessment and intervention
services for adults with Autism Spectrum Conditions (ASCs), Attention Deficit
Hyperactivity Disorder (ADHD).
Main duties of the job
The community learning disability nursing team is an integral part of
the neurodevelopmental services multi-disciplinary team facilitating and
supporting a wide range of health interventions including specialist health
assessments, transition work from child to adult services, Health Plans, Health
promotion and early intervention, Primary
and Secondary Care liaison and support, epilepsy management, dementia care, end
of life support, individual bespoke pathways, patient led care and much more.
In additions to working with our NDS multi-disciplinary team we work
closely with other services within YH such as District Nursing, Rapid Response to
coordinate holistic care and support our nurses to increase their skills and
knowledge.
We
