Job summary

The Physiotherapist will work in the community as well as with residents, service users and Care Home staff in assessing need, setting goals and reviewing outcomes. Main duties of the job

In addition, the successful candidate will be working to reduce the risk of deconditioning of care home residents and supporting facilitation of discharges from acute setting. You will work in partnership with the Community Physiotherapy Service, Care Home residents, Care Home staff, the Urgent Care & Support Service staff and other Health Care Professionals.

2. Job Purpose

3. Key Result Areas

KEY RESPONSIBILITIES

  • Proactively manage people in the care home setting to avoid unnecessary attendances at hospital and subsequent in-patient stays.
  • To perform physiotherapeutic assessments of residents with diverse presentations and complex physical and psychological conditions
  • To design and deliver individual physiotherapy treatment programmes based on a sound knowledge of evidence-based practice and treatment options using clinical assessment, reasoning skills and knowledge of treatment skills e.g., manual physiotherapy techniques, resident education, exercise classes and other alternative options
  • To facilitate residents and carers in the development of support plans, which meet their needs, outcomes and cover identified risks, offering the widest possible flexibility and choice allowing them to take control of their short-term goals
  • To evaluate resident progress through person-centred reviews and alter treatment programmes if required in partnership with other professionals in the multi-disciplinary team
  • To plan the discharge from the Urgent Care & Support Service in partnership with residents/ service users, their families and care home staff giving advice and information about their options and if necessary putting appropriate services in place
  • To facilitate service user choice and empowerment through promoting independence, in particular through the timely provision of information, advice and support to ensure service users maintain independent for as long as possible.
  • To ensure the highest level of data quality for all information recorded, ensuring that the information is timely, accurate and complete, to ensure the service meets all the statutory recording requirements.

To work as an autonomous practitioner and as a member of the Urgent Care & Support Service to be professionally and legally accountable for all aspects of their professional and clinical work within agreed national, local and professional standards and guidelines.

Undertake joint visits to service users in the care home with colleagues within the team and from other multi-disciplinary teams as directed/required.

Feedback effectively to members of the Urgent Care & Support Service team and external agencies on clients performance and progress towards identified exercise goals.

Respect the individuality, values, cultural and religious diversity of service users and contribute to provision of a service sensitive to these needs.

CLINICAL RESPONSIBILITIES

  • To be professionally and legally accountable and responsible for all aspects of own work including the management of individual caseload, working within accepted codes of professional conduct and clinical guidelines (where indicated), to minimise clinical risk.
  • Support the Urgent Care & Support Service and care homes with facilitating discharges from acute settings.
  • Ensure care homes provide baseline health data if the resident is admitted to hospital to support integrated, consistent care and facilitate discharge.
  • To recommend, determine and administer appropriate care plans utilising specialist treatment skills.
  • To identify appropriate care pathways and liaise / co-ordinate and facilitate multi-disciplinary and multi-agency referrals for clients with complex conditions and their families / carers.
  • Accept clinical responsibility for a designated caseload of patients and to organise this effectively and efficiently with regard to clinical priorities and use of time.
  • To provide specialist advice and guidance to other health or social care professionals, voluntary agencies and other organisations.
  • To formulate and be responsible for patient care plans carried out by assistant staff within health and social care services.
  • Provide advice in a variety of clinical and non-clinical situations, teaching and instruction to relatives, carers and other professionals, to promote understanding of the aims of physiotherapy and to ensure a consistent approach to patient care.
  • To be responsible for the competent use of clinical equipment, and patient appliances as indicated, and to report any defects or malfunctions to the senior clinicians/healthcare professionals.
  • Assess capacity, gain valid informed consent and have the ability to work within a legal framework with patients who lack capacity to consent to treatment.
  • To be responsible for maintenance of accurate written patient records within Organisational, Professional and Service protocols and guidelines. To complete any necessary documentation/reports for other professionals/agencies as required.
  • Use a range of verbal and non-verbal communication tools to communicate effectively with residents and service users to progress rehabilitation and treatment programmes
  • To provide a high standard physiotherapy service within the Urgent Care & Support Service, which involves working autonomously and as part of the multidisciplinary team
  • To work closely with the Urgent Care & Support Service lead/s and the other members of the team and follow up residents care as assessed by clinicians through the process of monitoring and evaluating to reflect the residents changing needs/condition
  • To prescribe and train use of rehabilitation equipment as appropriate.

Competent in training on recognising soft signs, vital signs, NEWS2 and escalation pathways

Demonstrate a high level of clinical judgement, acting autonomously to support care homes to interpret clinical signs

To undertake risk assessments at initial and subsequent visits to determine the safety or otherwise of the residents ability to receive their care within the care home, referring to the most appropriate service when required.

SERVICE DELIVERY RESPONSIBILITIES

  • To record accurate, legible clinical observations in a timely manner.
  • Delivers services in line with Better Standards for Health Care and relevant legislation.
  • Contribute to the development and implementation of guidelines, protocols and patient care pathways where required.
  • Act as a resource and educator for residents, carers, families, team members, GPs and other professionals within the MDT to provide and advise on evidence-based practices, including effective health promotional aspects, in order to fulfil requirements of local and national guidelines.
  • Be actively involved in the collection of appropriate data and statistics for the use of the service.
  • To take part in any integration and alignment programmes with our fellow provider services that are initiated by our commissioners.
