Job summary

The post holder will provide specialist physiotherapy skills within the community respiratory rehab programme. He/she will be responsible for the management and co-ordination of the community respiratory rehab programme for patients following referral by primary or secondary care.

Working closely with GPs, secondary care and community services, the community respiratory rehabilitation programme will provide patients with a service underpinned by BTS standards.The post holder will also work closely with Cardio-Respiratory team lead to support patients at home and at risk of hospital admission

Main duties of the job

To coordinate with the wider cardia-respiratory Specialist Physiotherapists supporting patients with long term respiratory diagnoses to manage their condition and optimise their health. Pulmonary Rehabilitation clinics & groups will be based in various hospital & community venues.

To triage, assess and deliver rehabilitation programmes in groups, remotely and in the patient's own home. According to local protocols and guidelines. You will also be required to support the team in other respiratory physiotherapy assessment and treatment sessions and deal with patient telephone queries and appointments.

To manage, assess and treat your own workload of patients and maintain associated patient records utilising knowledge and skills gained through attainment of competencies, experience and / or relevant qualifications. You will support in the delivery of Pulmonary Rehabilitation education talks on a wide variety of topics.

To undertake comprehensive assessment of people referred to the pulmonary rehabilitation service to determine their baseline and discharge clinical outcome measures within a variety of settings.To plan and deliver an individual specialist pulmonary treatment programme using knowledge and evidence-based practice and treatment options. The treatment plan is to be based on effective intervention and patient agreed goals taking into account their psychosocial needs.

4. Key Result Areas

1. To deliver the respiratory rehabilitation service in Kingston for patients with or at risk of developing respiratory disease and their carers/relatives in a safe environment and in liaison with stakeholders (CCG, GPs, Kingston Hospital).

2. To be accountable for the respiratory rehabilitation programme ensuring the highest standards are maintained.

3. To ensure that the respiratory service is of the highest standard and meet all national recommendations e.g. British Thoracic Society (BTS) standards NICE guidelines.

4. To be responsible for interpreting and implementing National Policies on respiratory rehabilitation ensuring that clinical practice changes in accordance with latest recommendations.

5. To have highly developed and extensive specialist knowledge and experience in respiratory rehabilitation, acting as an expert resource to GPs, the clinical team, allied health professionals, PCT and participating leisure centres.

6. To exercise a high degree of personal autonomy when assessing, managing and planning care for a case load of specific patients in different settings including patients homes.

7. To respond to assessment of patients needs in preventing admission to hospital and to liaise with relevant team members in admission prevention

8. Identify the need for training and organise and provide education and training to other health care professionals and outside agencies; PCT, community nurses.

KEY DUTIES AND RESPONSIBILITIES:

1. To deliver the community respiratory rehab programme ensuring the delivery of evidence-based care.

2. Implement appropriate referral pathways with primary and secondary care colleagues.

3. To work in conjunction with team members to undertake a comprehensive assessment and complex analysis of patients attending the programme, including all aspects of clinical, psychological, physiological and life-style factors, to formulate an individualised treatment plan using a problem-solving approach.

4. To provide specialist education and support to patients and their relatives, involving patients and their relatives/carers with all aspects of the care plan.

5. To manage own caseload, admitting and discharging patients to and from the service as appropriate.

6. To work with the respiratory nurse to teach the exercise component of the rehabilitation programme to groups of respiratory patients, bearing in mind some may have considerable co-morbidity adjusting and adapting the programmes to suit individual needs of the patient.

7. To promote a healthy lifestyle to the patient using and understanding recognised models of health promotion.

8. To liaise with Kingston GPs and other partners to identify and recruit patients with lung disease.

9. To develop strong links with local services post pulmonary rehabilitation.

10. To identify patients with lung disease and work with the multidisciplinary team on their treatment in the community.

11. In partnership with other members of the Cardiac and Respiratory Integrated Service (CRIS) team, deliver health education and lifestyle advice, to empower patients and relatives to make informed choices about their health and well-being and increase concordance with suggested therapies and lifestyle changes.

12. To use developed motivational and negotiation skills to facilitate acceptance and ongoing compliance with treatment programmes where there are complex barriers to acceptance and difficulties in understanding due to cultural background. (This may also require the use of interpreters and/or non-verbal communication aids.)

13. To inform patients of the benefits and risks of suggested treatment options

14. To provide support for patients and relatives where the diagnosis of lung disease has serious repercussions/limitations on quality of life e.g loss of job, using highly developed reassurance, and empathetic communication skills.

15. To encourage patients to set realistic targets in their lifestyle modification.

16. To respond to deterioration in a patients condition to influence the management plan.

17. To develop pathways / referral links into community programmes following the completion of the pulmonary rehabilitation.

18. To explain and interpret relevant investigations and explain the results and significance to the patient.

19. To provide qualitative and quantitative data to support the auditing of the programmes effectiveness and to participate in the PRSAS data collection.

20. To communicate with GPs and other health care professionals the outcome of the patients rehab in a timely fashion using written, personal, telephone or electronic contact to ensure continuity of care.

21. Develop and maintain professional and educational links with cardiac and respiratory specialists, cardiac rehabilitation and respiratory rehabilitation HCPs and district nurses in the local network, building professional partnerships with colleagues.

22. Use clinical judgement in situations where no guidelines / policies exist.

23. To act as a resource within the community, developing best practice alongside colleagues working in general practice and work with others to assess the impact of the pulmonary rehabilitation on health and wellbeing.

MANAGERIAL RESPONSIBILITIES

5. To ensure the implementation of clinical governance frameworks and quality procedures within the team; to ensure risk management strategies are in place and apply to all aspects of the work situation and ensure others do likewise.

6. To be responsible for selecting suppliers for the procurement of equipment for the pulmonary rehabilitation programme and to give advice on appropriate equipment for use with respiratory patients.

7. To facilitate good communication between the multidisciplinary team members via regular team meetings, verbal and written reporting.

8. To participate in the regular meetings which oversee the delivery and development of the service and to represent the community pulmonary rehabilitation service and other health professionals, as required, within relevant meetings
