2.Role of the department.
To provide specialist health assessments and interventions to meet the health needs of adults with learning disabilities in the London Borough of Richmond upon Thames and the Royal Borough of Kingston upon Thames.
Working in collaboration with other health professionals and agencies to provide advice, training and consultancy in order to establish consistent good practice.
3. Job Purpose
To provide high quality skilled nursing input, carrying out on-going assessment, planning, implementation and evaluation of the holistic care needs of people with learning disabilities, their families and/or carers.
The post will contribute on the whole to the activity of the Neurodevelopmental specialist healthcare service for people with learning disabilities working across Richmond and Kingston boroughs.
4. Key Result Areas/Principal Responsibilities:
4.1. Clinical
To act as a specialist nurse providing advice and appropriate training to service users, their carers and health and social care colleagues as needed.
To be personally accountable for professional and ethical actions and ensure compliance with NMC code of conduct.
To take on a link nurse role between the Learning Disability Service and specialist health services i.e. health screening series, mental health and hospital services.
To maintain up to date and accurate records, ensuring confidentiality is maintained in accordance with Your Healthcare policies.
To provide a high standard of skilled nursing input for people with learning disabilities in a range of settings (own home, residential care, supported living, hospital) in compliance with the relevant protocols and guidelines.
To undertake a range of assessments and interventions for people with learning disabilities and complex health needs.
To support transition from Child to adult service
To coordinate follow up nursing input with complex referrals to access primary, acute and specialist health care services in accordance the individual assessed needs of the client.
To empower clients, their families and carers to influence and participate in decisions concerning their health needs by providing information on a range of applicable interventions and service options.
To provide accessible clinical advice and guidance to people with learning disabilities and families in a person-centered way.
To communicate observations of physical and or mental health concerns as well as social concerns to other members of the multi disciplinary team as appropriate.
To provide health promotion activities in line with Your Healthcare, local and national priorities.
To support individual service users ensuring their rights and choices are respected in order to achieve optimum health outcomes.
Actively promote a healthy and equitable approach to risk management and contribute / lead on developing risk management strategies where appropriate.
To be responsible for alerting other agencies to those clients most at risk via appropriate referral systems.
To have a sound understanding of the Mental Capacity Act (2005) and implement principles of capacity, consent and best interests in own practice, and in others if taking lead responsibility for individuals care.
4.2. Service Delivery
To record all work undertaken in line with NMC guidance and local policy and procedure
To contribute to the development of policies and protocols for the Neurodevelopmental service in the community that will impact on all service users.
Contribute to the development and implementation of guidelines, protocols and patient care pathways for use across the Neurodevelopmental service
Be responsible for ensuring the effective selection and use of all treatment resources available.
To have a sound working knowledge of safeguard adults from abuse and its application in practice.
To be able to work across both community based teams of the Royal Borough of Kingston upon Thames, and the London Richmond Borough upon Thames in accordance with Your Health Care requirements.
To be actively involved in the Learning Disability Mortality Review (LeDer) Programme, partaking in reviews and implementations of learning gained from the process.
4.3. Management
To participate in one to one clinical supervision, peer supervision and appraisal in accordance with Your Healthcare policy and contributing to a personal development plan.
To prioritise own workload and manage own caseload effectively.
Support multidisciplinary team working, by facilitating colleagues to effectively prioritise, plan and manage their caseloads
To mentor pre-registration students while on placement with the team.
To supervise or oversee the work of junior staff
To support staff induction
4.4. Quality
Be actively involved in the collection of appropriate data and statistics for the use of the service.
To undertake and or participate in audit activity to ensure clinical effectiveness and best practice is achieved.
To comply with current Evidence Based Guidelines, Your Healthcare and relevant local policies and procedures
To contribute to research and or audit to further develop clinical practice and or specific project work.
To contribute to the development, evaluation and monitoring of the teams operational policies and services, through the deployment of professional skills in research, service evaluation and audit.
