These posts are part of our
integrated Community Therapy Service ,
based within our Occupational Therapy Team at our Headquarters
in Hollyfield House, Hollyfield Road, Surbiton. We have 2 posts one of which can be based
at our Rehabilitation Unit at Teddington Hospital ( Cedars at Grace Anderson) with
some community cover , the other would
be based at Hollyfield House ,with some Rehabilitation Unit cover .
As part of the broader multi-disciplinary Your Healthcare community
services the Occupational Therapy service provide assessment and rehabilitation
programmes to patients coming out of acute hospital care to home or our
Rehabilitation Unit , or needing support to stay in their community residence. The service continues to expand to support
National model of care and is now part of the 7 day provision from Community
Services , operating from 08.00-20.00.
Some shift and rota work may be needed including weekends.
Main duties of the job
The post holders would ideally
have a clinical grounding in general community working with a caseload
predominantly from the care of the elderly population, Experience of acute work
would also be of benefit. They would need to be organised, flexible and highly
motivated to work with our service to deliver best possible outcomes for our
patients.
As part of the OT team the
post holders would be working within a broader inter-disciplinary team and will
have supervision from experienced existing OT colleagues. The post offers the
potential for a wide variety of experiences, depending on skills and interests.
The Occupational Therapy team are
part of the Therapy Services so have links with OT colleagues working across
the Borough in other organisations including Social Services, Acute Hospital and Mental Health.

Job responsibilities
1. Job Purpose
·
To work as a member of the Your Healthcare Integrated
Occupational Therapy team to provide an Occupational Therapy Service to
patients who may have complex acute, chronic, or multiple pathology
presentations requiring assessment and rehabilitation either as an in-patient
or in the community setting.

To formulate goal focussed treatment plans and
delegate appropriately to technical instructors and generic care assistants
who work as members of the multidisciplinary team.
·
To work as a member of a multi-disciplinary
team, ensuring the communication of information to all health and social care
agencies involved, to meet the holistic needs of the patient.
- To
contribute to the of planning, coordination, delivery and evaluation
of Occupational Therapy services
provided in the rehabilitation services.
- To
provide supervision and support to less senior staff, assistants, other
professionals and act as clinical educator for undergraduate students.

Role of Directorate
To ensure the provision of high quality multidisciplinary
community services to the population served by Your Healthcare that aims to
enable people to access the right care at the right time delivered by the most
appropriately trained professional.
The directorate will further develop the strong
partnerships with primary care, social care, the voluntary sector and acute
hospitals
- To
flexibly respond to control demand on acute hospitals by proactively
managing people in the community to avoid unnecessary attendances at
hospital and subsequent in patient stays.
- To
facilitate timely discharge of patients either to their own home or into
an appropriate setting.
- To establish more integrated adult and
OT services across Reablement/ Intermediate Care, Rapid Response ,
Cedars at Grace Anderson and Community Services, including Kingston
Hospital , as required.
The directorate will pro
actively review and transform services to meet the needs of the population
adopting evidence based good practice and ensuring the efficient delivery of
services that clearly demonstrate value for money. The Your Healthcare Integrated
Occupational Therapy team is part of the wider Your Healthcare community
provision and as such will be seeking
ever stronger links and shared pathways with its colleagues in other
community and acute provider organisations.
Key Result Areas
4.1. Clinical
- To be professionally and legally accountable
and responsible for all aspects of own work including the management of
individual caseload, working within accepted codes of professional conduct
and clinical guidelines (where indicated), to minimise clinical risk.
- To undertake comprehensive assessments and
formulate accurate diagnoses and prognoses in a wide range of conditions
requiring rehabilitation, recommending the best course of
intervention. This will include a wide
range of conditions found in the care of the elderly spectrum .
- To draw upon clinical reasoning, analytical
and communication skills to inform assessments. This will include the use
of highly specialised standardised and non standardised assessment tools
(physical and cognitive), observations, questionnaires,
multi-agencies/professional information gathering to determine an
appropriate treatment plan
- To assist with timely patient discharge and
access to community care pathways from Kingston Hospital as part of the
Integrated Community OT service in-reach
to Kingston Hospital.
- Utilise physical, cognitive and sensory
assessment batteries and techniques requiring sound knowledge of
assessments, eye hand co-ordination, dexterity, physical ability (ie manual
handling) and task analysis.
- Formulate and implement occupational therapy
treatment plans based on current evidence and clinical reasoning with an
understanding of available treatments including client education, skills
development/rehabilitation and compensatory techniques or use of medical
devices to support an improvement in quality of life and/or client
independence.
- To act as a source of occupational therapy
clinical expertise and advice providing clinical and professional
leadership within the rehabilitation services and as a resource to
colleagues in other teams i.e. social services.
- To maintain and monitor own, and others,
professional standards in accordance with College of Occupational
Therapists guidelines and standards
- To assess the patient’s understanding of the
treatment proposals to ensure valid informed consent is obtained ensuring
practice is within the legal framework for those unable to consent to
treatment themselves.
- To apply knowledge and skills in assessing
feasibility of home adaptations, in order to recommend major adaptations
to social care colleagues, that will meet clients and families complex
health and social care needs.
- As a member of the multidisciplinary team
works to proactively manage people in the community to avoid unnecessary
attendances/admissions to hospital and subsequent in patient stays.
- To identify appropriate care pathways and
liaise / co-ordinate and facilitate multi-disciplinary and multi-agency
referrals for clients with complex conditions and their families / carers.
- Accept clinical responsibility for a
designated caseload of patients and to organise this effectively and
efficiently with regard to clinical priorities and use of time.
- To formulate and be responsible for patient
care plans carried out by assistant staff within health and social care
services.
- Provide spontaneous and planned advice,
teaching and instruction to relatives, carers and other professionals, to
promote understanding of the aims of occupational therapy and to ensure a
consistent approach to patient care.
- To assess for the provision of necessary
equipment and/or minor adaptations and check that equipment complies with
Health and Safety standards and to legal requirements, as well as to
demonstrate the safe use of equipment/adaptations to clients and carers.
