Job summary
You will provide exercise based support and advice to patients providing them with a structured evidence based home exercise plan and be based between Eye and Bury. All patients will have been through a clinical triage process before being referred to your care. You will be treating patients who have been diagnosed with either single or multiple health conditions such as musculoskeletal injuries or pathology, type two diabetes, long COVID symptoms or patients with persistent pain. Patients will be cleared from any underlying health conditions that adversely impact their ability to complete exercise based treatment effectively and safely by a Physiotherapist. You will be provided with a detailed referral for each patient including their diagnosis, goals and outcome expectations. Sessions will be predominantly class based or if appropriate as one to one sessions.
Main duties of the job
- To work in partnership with AHP Suffolk physiotherapists to enable and empower individuals, families and communities to engage in healthy lifestyles as part of service development.
- To monitor the condition of the patients during classes and one to one sessions and ensure they exercise in a safe and appropriate way.
- To ensure the workspace you are assigned to is safe and secure for staff and patients, including any digital systems.
- To participate with the delivery of planned programmes of care to participants with NHS based treatment.
- To implement treatment plans and progress them with the support of the referring Clinician and other members of the Exercise Therapy team.
- To monitor the progress of patients receiving exercise based treatment under your care and communicate relevant changes back to the referring Clinician or senior staff member as appropriate.
- To prepare the treatment area prior to a class or one to one session including visual safety checks, tidying and wiping down, ensuring appropriate equipment is in place and ensuring a comfortable and safe environment.
- To assist in the day to day administration of the clinical pathways including diary management, preparing documents for sessions, photocopying, filing, data management including maintenance of patients details and use of bespoke IT Systems.
