Job summary
This Programme and Performance Management role is focused on the delivery of the Discharge to Assess Programme in Bristol, North Somerset and South Gloucestershire (BNSSG). Working to a Programme Director, based within Sirona Care and Health, and drawing on support from within the Integrated Care System, the successful applicant will have outstanding communication skills, be able to digest complex information swiftly and accurately, and be able to engage a wide range of stakeholders.
Experience of working in a local authority or community health setting would be of benefit.
Main duties of the job
Discharge to Assess is a national initiative that sees people that have experienced a hospital admission discharged home for assessment of their longer term health and care needs. This directly improves the experience that people have and helps to avoid poor outcomes associated with unnecessarily long hospital stays. The programme of work to deliver the changes required has been live for some time and the post holder will be joining a well-established team of community and social care providers who are working collaboratively on the redesign and delivery of new service models across the BNSSG area.
