Job summary
Skylark Rehabilitation Unit is a 30 bedded facility in Yate (South Gloucestershire) which opened in February 2018. The unit is one of several community-based units providing active rehabilitation and ongoing nursing care for those people being supported on Discharge to Assess pathway 2. (Please see below).
Following a hospital stay, the ethos for all people being discharged is Home is best and a large number of people are supported to return home utilising the robust and established Discharge to Assess Pathway 1. For those who have on-going rehabilitation needs that are better met in a bedded unit before returning home, we have a number of community settings that provide a step down to a Rehabilitation, Reablement and Recovery bed (pathway 2). This will include Skylark Rehabilitation Unit.
During their stay, those people we support will experience intensive therapy intervention in our nurse and therapy led unit, with support given to them, their carers and families around complex discharge planning. This will enable a timely and safe discharge to the persons usual residence or to a more suitable location if appropriate.
We actively promote a multi-disciplinary mode on the unit where nursing staff work closely with therapists, social workers, GPs with special interest in older peoples care as well as other acute and community colleagues, to ensure the person we are supporting is kept at the heart of every decision.
This role is for 23 hours, nights.
Main duties of the job
- With the support of senior staff be responsible for implementation and evaluation of programmes of care for patients who are acutely and chronically unwell; often with complex needs arising from the frail elderly cohort.
- Use clinical reasoning skills to assess, plan, implement and evaluate, patient- centred intervention programmes using an evidence based practice approach.
- Collaborate and negotiate with service users, carers and family in identifying appropriate and achievable goals as part of the overall care plan.
- Work within a multi-disciplinary team, to provide integrated skills and sharing knowledge providing holistic and specialist skills supported by a robust competency framework
- Promote a team approach to care by ensuring effective and appropriate networks with other members of the primary, secondary health care team and other agencies.
- Supervise members of the team including peers, students and unregistered workforce in nursing competencies.
- Actively participate in training; this may involve assisting in the delivery of training within the service and to other health professional as appropriate. Support the development of additional competencies within the requirements of the multi-disciplinary approach of the unit, following appropriate training.
- Identify risks and act accordingly to minimise risk to self, service users and colleagues.
- Work flexibly to meet the service needs and undertake any other duties that may be reasonably requested to help the service run smoothly.
