Job summary

Then we would like you to join one of our Integrated Network Teams (INTs) in the Bristol North and West Locality as a Senior Community Nurse.

Our INTs plan and deliver evidence-based nursing care to housebound patients with both short and long term nursing and therapy needs within defined geographical areas. You will work both autonomously and collaboratively within supportive teams.

You will have excellent communication and clinical skills and be able to demonstrate previous experience of leading and working within a team. Additionally you will possess excellent leadership, managerial and IT skills, alongside good time management and the ability to prioritise your own workload. We will provide you with ongoing professional development and support from our leadership team.

Main duties of the job

  • Responsibility for all aspects of service user management in the team, planning day to day care influencing changes to clinical practice and being a role model.
  • Ensure collaboration of the team with other health professionals.
  • Coordinate staff, ensuring appropriate staffing levels and appraising, developing and leading.
  • Ensure all clinical practice within the team is evidence based.
  • Manage own case load, carrying out a range of clinical activities.
  • Deal with a variety of information, managing daily team priorities, keeping these under review and working with other teams to meet objectives.
  • Contribute to the development of new policies/practices and help in their introduction and possible adaptation.
  • Oversee a range of systems, equipment and devices that help provide coordinated care.
  • Assist in further integration of health and social care practices, breaking down barriers and enabling more joint working with other practitioners and teams.
  • Seek ways to improve and extend services; make recommendations, partake in audits and contribute to tendering processes to retain/obtain new services.
  • Resolve problems, using analytical/problem solving techniques. Use judgement to select best course of action.
  • Influence the team by demonstrating best practice and help shape the team culture.
  • Chair/attend decision making panels.

Essential

  • Hold recognised qualifications with the professional area at degree level/BTEC Advanced Professional Diploma/Graduate Certificate Level/NBQ4/VQ4.
  • Hold a degree or qualification recognised as equivalent by professional body and with the appropriate registration.

Desirable

  • Hold or working towards a post-graduate a post graduate diploma/degree in professional disciple and/or management practice.

Experience

  • Previous post qualification experience working in a health and social care environment within the relevant professional discipline.
  • Depth of knowledge of principles and practices within a professional discipline.
