Our Community Nurses plan and deliver evidence-based care to patients within their own homes and clinics. You will work collaboratively as part of a supportive team comprising of Community Nurse Team Leaders, other Community Nurses, Therapists, Support Workers and Administrators as well as with the wider healthcare community.

You will have access to a comprehensive training package to enable you to meet the increasing challenges of complex care in the community. You will need excellent communication, time management and clinical skills as well as experience of working within a team. You will be required to use electronic patient record systems, and prioritise your own workload.

We can offer a variety of shifts between the hours of 8am-8pm, 7 days a week, supporting flexible working to give you a good work-life balance.

Main duties of the job

  • Undertake the full range of clinical activities of the role, having received training, and been assessed as competent.
  • Complete the appropriate training and Sirona Band 5 nursing framework competencies in order to deliver a competent clinical service.
  • Escalate any serious changes in patient presentation or concerns you have to a more senior member of your team.
  • Use shared decision making to empower patients and agree a person centred care plan - ensure implementation, evaluation and review of the plan.
  • Make clinical judgements and decisions - can often require analysis of complex facts and situations.
  • Monitor long term conditions, as delegated and in line with protocol.
  • Communicate effectively with patients and other stakeholders including communicating sensitive information concerning the patients medical condition and writing clinical records.
  • Provide mentorship, supervision and training to non-registered staff and peers.
  • Take responsibility for own clinical competence and professional registration.
  • Undertake audit activities and participate in research developments to improve the quality and safety of the service.
  • Promote integrated care by supporting a team approach; ensuring effective and appropriate handover and communication with team members, other members of the primary, secondary health care team and other agencies.

  • Registered Nurse with the NMC (Level 1)

Desirable

  • Mentorship and supervision qualification or working towards

Experience

Essential

  • Ability to effectively communicate complex clinical information to a diverse patient group
  • Ability to manipulate fine tools e.g. for intravenous injections, syringe pumps, insertion of catheters etc
  • Competent IT skills email, Microsoft Office, bespoke clinical systems
  • An understanding of the issues of equality and diversity
  • Knowledge of how healthcare systems are commissioned and provided
  • Understanding of primary care and key national local priorities for community nursing

Desirable

  • Previous experience as a Band 5 Nurse, preferably in the community
