The Sirona Integrated Network Teams (INT) are an innovative and dynamic service. Our therapists support hospital discharges, admission avoidance and long term self-management, enabling patients to live better lives in their home environment. 

We are seeking an enthusiastic and motivated Senior Occupational Therapist to join our busy and dynamic team, supervising colleagues and promoting independence and choice for our service users and their carers.

You will be working within a friendly and supportive team alongside Physiotherapists, Occupational Therapists, Nurses, Advanced Clinical Practitioners, Support Workers and Administrators. We collaborate closely with our Specialist Services, Acute, Community and Social Care partners. We offer a flexible service which covers extended hours operating 7 days a week, to support our service user needs.

Working arrangements include weekends and bank holidays on a rota basis.

Main duties of the job

  • Responsible for the initial holistic assessment, implementation and evaluation of programmes of care for patients who are acutely and chronically unwell; often with complex needs.
  • Use clinical reasoning skills to assess, plan, implement and evaluate, patient-centred intervention programmes, using evidence based practice approach.
  • Responsible for allocation of work and team coordination.
  • Develop strong, communicative working relationships with hospital staff, primary care, service providers and multi-disciplinary teams.
  • Collaborate and negotiate with service users, carers and family in identifying goals as part of a care plan to maximise quality of life and independence.
  • Facilitate appropriate referrals to other services or investigations.
  • Focus on achieving and demonstrating overall improvements in health care for patients who are complex/frail, have urgent care needs and long-term conditions
  • Use occupational therapy skills in functional and standardised assessment.
  • Assess and arrange the provision of equipment and adaptations from appropriate agencies ensuring this is in good working order.
  • Take responsibility for development of members of the INT including line management duties whilst assisting to organise and manage their caseload.
  • Participate in training; delivering as appropriate.
  • Identify risks and act accordingly to minimise risk.
