We have an exciting opportunity and are seeking an experienced leader to come and lead our Integrated Network Team. Some of the key responsibilities are to oversee the delivery of:

  • Planned nursing pathway (formally known as District Nurses).
  • Prevention of Admission pathway: provision of an urgent community response including clinical assessment of deteriorating patients in their home environment; supporting patients to remain at home if clinically appropriate.
  • Discharge to Assess Pathway 1 (DtA P1): enhanced community care to facilitate discharge from hospital

Main duties of the job

  • Demonstrate and provide inspiring and innovative leadership that supports the implementation and development of high quality integrated person-centred care which respects and responds to the needs of service users and carers.
  • Hold overall strategic management, business planning and development of the teams within your Locality, in conjunction with the Associate Locality Director and supported by three Assistant Locality Managers and two Integrated Network Team Managers.
  • Ensuring delivery of KPIs and support ongoing transformation of teams within South Gloucestershire, working closely with other Locality Managers from across the BNSSG area.
  • Be one of our CQC Registered Managers, coordinating activity and delivery against KPIs and contract reporting, consult directly with stakeholders, service users, carers and members of the public. Using a continuous improvement methodology, be responsible for evaluation of clinical effectiveness, using relevant data/information, identifying new risks and reviewing ongoing ones.
  • Work closely with our neighbouring localities and our experienced corporate services (e.g. Finance, Communications, HR, BI etc). Have a problem-solving aptitude and ability to prioritise your own work whilst supporting the priorities of your direct reports.

  • Professional qualification in Health Care and current registration.

Desirable

Experience

  • Extensive operational management experience including experience of managing and developing multi-disciplinary teams in a variety of locations.
  • Expert knowledge in more than one specialist field and good understanding of several related disciplines.
  • Previous experience working in a health and social care community setting and/or other relevant multi-disciplinary environment of similar size and complexity.
  • Knowledge and understanding of safeguarding policies and procedures for children and adults, and experience of supporting front line staff in delivery.
  • Demonstrate good understanding of the transformation agenda and the legislative, regulatory and performance management framework for health and social care.
  • Demonstrate leadership skills, credibility, integrity and ability to manage through change.
