We're looking for a Senior Occupational Therapist to join our South Bristol team, delivering person-centred care in the community.

This is a great opportunity to support people to live well at home--preventing hospital admissions, enabling timely discharge, and delivering meaningful rehabilitation that improves quality of life.

Our Integrated Neighbourhood Teams are forward-thinking and focused on personalised care. You'll work alongside OTs, Physiotherapists, Nurses, Specialist Practitioners and Support Staff to deliver high-quality care in patients' homes.

Main duties of the job

Deliver holistic assessments, planning and evaluating care for patients with acute and complex needs
Use clinical reasoning to provide evidence-based, person-centred interventions that promote safe, high-quality care
Manage your own caseload and support effective team coordination and service delivery
Work proactively with MDT colleagues to prevent avoidable admissions and support timely discharges
Collaborate with patients, families and carers to set meaningful goals that maximise independence and quality of life
Facilitate referrals and coordinate care based on individual needs
Assess and arrange appropriate equipment within scope of practice
Support, supervise and contribute to the development of junior staff, peers and students
Maintain accurate, timely and legally compliant records
Contribute to training and continuous service improvement
Identify and manage risk to ensure the safety of patients, colleagues and yourself
