Job summary
The number of service users who have problems with adherence and need support to take their medication is growing. With support from The Health Foundation, BNSSG ICB have secured funding for a pilot of a personalised medication support service (PMSS).
This role will support the delivery of this pilot project in Sirona Care and Health. The post holder will work closely with the Sirona Medicines Optimisation Team to trial how people on the Sirona caseload could manage their medication independently with the support of technology.
They will visit potential trial participants in their own homes, provide personalised information on technology options available to support self-management of medications and, if appropriate, install technology options. Throughout the trial period they will follow up participants to assess the effectiveness of the interventions.

Main duties of the job
- Provide personalised information and assistance in the use of technology to support people to self-manage their medications.
- Work in partnership with the University of the West of England (UWE), BNSSG Integrated Care Board, the Local Authority Technology Teams and Sirona Care and Health, to deliver and evaluate a personalised medication support service.
- Conduct home visits to assess and advise on the most appropriate technology, agree technology package with service user, support the installation and assess the effectiveness of the interventions.
- Effectively manage and prioritise a caseload, visiting service users in hospital or at home, as required.
- Deliver specific performance targets and project objectives, as agreed with the relevant line manager.
