Skylark Rehabilitation Unit is a 30 bedded facility in Yate (South Gloucestershire). The unit is one of a number of community units providing support and active rehabilitation for those people being supported on Discharge to Assess Pathway 2.
You will work as part of our multidisciplinary team, all working together, to deliver individualised nursing and rehabilitation care to patients with the common goal of delivering the best outcomes possible for our patients.
Our patients present with a broad range of conditions including orthopaedic, frail elderly, post-surgery and long term neurological conditions and we work with them to help them attain the highest level of personal independence prior to discharge.

Main duties of the job
The unit nurse is responsible for the assessment, planning, implementation, and evaluation of care for a group of identified service users and will provide and maintain a high standard of patient focussed care that ensures patient safety; dignity and respect are paramount to the care delivered.
- Provide, monitor and ensure that a high standard of safe and effective nursing care is maintained via professional actions, verbal and written skills.
- With the support of senior staff be responsible for the initial holistic assessment, implementation and evaluation of programmes of care for patients who are acutely and chronically unwell; often with complex needs arising from the frail elderly cohort.
- Carry out a range of routine and non-routine tasks including medication rounds, clinical observations, planning, implementing, and evaluating care plans, assisting patients to meet their daily care needs, health promotion, venepuncture, planning discharges for patients.
Work Pattern
You will be expected to work weekends and bank holidays on a rostered basis. Day and night shifts but predominantly day shifts.
