Job summary
We are excited to be in the position to recruit a Band 5 Occupational Therapist to join our rotation.We are looking for enthusiastic Occupational Therapists who are seeking a role where they can work in a supportive environment to assess patients with a variety of conditions, design andimplement rehabilitation plans and have an active involvement in discharge planning .Currently the post is rotational between the Pathway 2 rehabilitation units at South Bristol Community Hospital and Pathway 3 beds in various nursing homes within the Bristol area. The postholder will need to be able to work in all of our Pathway 2 Bed Bases (South Bristol Rehabilitation Unit and Sub Acute Stroke Rehab Unit) with appropriate induction to each service. It is anticipated that there may be further rotations added to this post in future.We are passionate about rehabilitation and the ability to empower service users to regain their health and independence following illness or injury. You will have the opportunity to work ina variety of multi-disciplinary teams and provide assessment and rehabilitation in a variety of settings.
Main duties of the job
To provide a safe, patient centred, effective and evidence-based Occupational Therapy service inthis setting
To provide specialist assessment, rehabilitation and management of people with acute and long termconditions including the promotion of health and prevention of secondary complications.
You will be part of an integrated multi-disciplinary team (MDT), which includes Nurses,Physiotherapists and other Allied Healthcare Professionals
You will work closely with case managers and/or social care providers to facilitate safe and timelydischarges
To work variable days and shifts over 7 day period between the hours of 08.00 - 20.00 in order tomeet the needs of the service.
In order to provide organisational and service resilience, you may be expected to support other teams or services, both within the locality and Sirona wide when the need arises at short notice working on a rota basis
With the support of senior staff be responsible for the initial holistic assessment, implementation
