Job summary

The Community Respiratory Service delivers pulmonary rehabilitation programmes, Home Oxygen Assessment Service and supports the NHS@home respiratory pathway.

We also provide an Advice and Guidance Service for healthcare professionals across BNSSG from across the care pathway.

Our service supports people with long term respiratory conditions living in the community to help them manage their condition and improve their quality of life.

The Community Respiratory Service sits within Specialist Services and provides support to adults across Bristol, North Somerset and South Gloucestershire.

  • Our service hours are 8.00am -6.30pm, seven days a week.

  • Our team is based at new Friends Hall, Stapleton, BS16 - but expected to travel to patient's homes, clinics and community venues across the Bristol, North Somerset and South Gloucestershire (BNSSG) area

Main duties of the job

As the post holder you will be responsible for provision of a patient-centred, evidence-based service to patients within the Bristol, North Somerset and South Glos area (BNSSG) and will provide:

  • Expert clinical assessment, advice and treatment for people with chronic respiratory disease within the community setting.

  • Support and deliver Pulmonary Rehabilitation programmes and the Home Oxygen Therapy service.

  • Provide specialist assessment for patients on the Acute Respiratory at NHS@Home Pathway

  • Be a specialist resource for colleagues across both professional and organisational boundaries sharing expert knowledge to ensure effective medical management and integrated working.

  • Undertake expert specialist respiratory assessment of services users, including those with complex presentation, to formulate individualised management and treatment plans using evidence based clinical reasoning skills and utilising a wide range of treatment skills and options.

  • To perform specialist respiratory assessments (including assessment for home oxygen therapy) in a variety of settings by undertaking a holistic approach and following local and national guidelines and protocols.

  • Work with service users and their carers towards shared goals providing advice and support, signposting to other community services and organisations to ensure person centred care and promoting physical, psychological and social wellbeing.

  • Provide education and advice to enable self-management and prevent unnecessary hospital admissions.

  • To support the service in carrying out Pulmonary Rehabilitation programmes.

  • To work with patients to decide on the most appropriate PR pathway for them, working as part of an integrated team and in partnership with other health care providers.

  • Be a proactive member of the multi-disciplinary team and have a significant role in the on-going service development, delivery and evaluation of the Community Respiratory service across BNSSG.

  • Co-ordinate the day-to-day organisation of the multidisciplinary team to ensure the service is responsive to the needs of the service users and other health care professionals needing advice and guidance.

  • Ensure effective delivery of service and adhere to all policies of the organisation by prioritising, organising and delegating appropriately.

  • Contribute to setting standards of care and auditing clinical outcomes and service provision.

  • To maintain and enhance standards of patient care, through the implementation of continuous quality improvement initiatives, regular service evaluation and development that involve patients/users of the service.
